چکیده مقاله:

Introduction Respiratory distress syndrome (RDS) is a current cause of morbidity in premature infants resulted from surfactant deficiency. The primary aim of this randomized study was to describe the feasibility of early administration of surfactant via a thin catheter during spontaneous breathing (TEC) and compare its outcomes with the InSurE (Intubate, Surfactant, Extubate) procedure Materials and Methods Premature infants with RDS, who were ≤ 32 weeks old and stabilized with nasal continuous positive airway pressure (nCPAP), were randomized to receive surfactant either by the TEC or InSurE technique. Tracheal instillation of 200 mg/kg Curosorf via 5-F catheter during spontaneous breathing under nCPAP was performed in the intervention group (n=38). In the InSurE group (n=40), infants were intubated, received positive pressure ventilation for 30 seconds after surfactant instillation, and placed on nCPAP immediately Results Necrotizing enterocolitis rate was significantly lower in TEC group, than InSurE group, [p

A Comparative Study of the Efficacy of Surfactant Administration through a Thin Intratracheal Catheter and its Administration via an Endotracheal Tube in Neonatal Respiratory Distress Syndrome

Background: The cornerstone of the treatment of respiratory distress syndrome (RDS) is respiratory support and surfactant replacement therapy. The administration of surfactant through a thin intratracheal catheter is one of the methods used to reduce one of the standard technique complications of a surfactant injection (Intubation-Surfactant-Extubation method [INSURE]). The aim of this study wa...

Introduction  Mechanical ventilation may be lifesaving intervention,It can be associated with complications,Thus, successful weaning is constitutive. One of the factors which is  important in successful weaning is  method of weaning. It is shown that  weaning is conducted successfully by using Spontaneous Breathing Trial (SBT) through T-piece and pressure support (PS) ventilation. But few studies ...

Zinc Supplementation during Pregnancy: A Randomized Controlled Trial

Zinc deficiency during pregnancy has been related to adverse pregnancy outcomes. However, the results of zinc-supplementation trials have not been consistent in improvement of pregnancies outcomes. This study was undertaken to investigate whether zinc supplementation was associated with pregnancy complications in Iranian women during the last 2 trimesters. It also assessed the anthropometric me...

A randomized controlled trial of the laryngeal mask airway for surfactant administration in neonates.

OBJECTIVE To compare the short-term efficacy of surfactant administration by laryngeal mask airway versus endotracheal tube. METHODS Preterm infants (28-35 weeks of gestational age), weighing 1kg or more, with respiratory distress syndrome, requiring nasal continuous positive airway pressure, with increased respiratory effort and/or fraction of inspired oxygen (FiO2)≥0.40 to maintain oxygen s...
