Repair of the volar scapholunate ligament has not been performed arthroscopically. We present an all-inside technique that presents closure of the anterior scapholunate interval. A Tuohy needle and a resorbable suture are all that is required.
Five cases of scapholunate instability are reported. The condition is commonly misdiagnosed in accident and emergency departments. The importance of a complete clinical assessment of the suspected scaphoid injury and the need to measure the scapholunate distance and the scapholunate angle on the radiographs is stressed.
Seven patients with chronic scapholunate instability (Geissler grade 2-4) were treated by percutaneous placement of screws across the scapholunate joint after arthroscopic debridement of the remnants of the scapholunate ligament. In all seven cases, the screw caused partial destruction of the lunate and/or scaphoid requiring screw removal within 6 months. We no longer perform this procedure.
