A 57-year-old female from Ghana was found to have fungating breast lesion and treated for breast cancer with empirical chemotherapy in her home country. On presentation to our breast surgical department in the UK, triple assessment and core biopsy showed the fungating mass to be a caseating granuloma in keeping with Tuberculosis infection.

A case is described of primary biliary cirrhosis in a 59 year old woman who initially presented with a rash over her lower legs 18 months before diagnosis. Skin biopsy examination showed non-caseating granulomas of the sarcoid type. It is believed that this is the first reported case of primary biliary cirrhosis presenting with a granulomatous rash.

However, although depressed cell-mediated immunity is commonly described, it is extremely rare to find a humoral immune paresis. A 13-year-old boy with severe, active granulomatous ileocolitis presented with bilateral conjunctival nodules and infiltrates, iritis, and uniocular corneal infiltrates.
