A 57-year-old female from Ghana was found to have fungating breast lesion and treated for breast cancer with empirical chemotherapy in her home country. On presentation to our breast surgical department in the UK, triple assessment and core biopsy showed the fungating mass to be a caseating granuloma in keeping with Tuberculosis infection.

A case is described of primary biliary cirrhosis in a 59 year old woman who initially presented with a rash over her lower legs 18 months before diagnosis. Skin biopsy examination showed non-caseating granulomas of the sarcoid type. It is believed that this is the first reported case of primary biliary cirrhosis presenting with a granulomatous rash.

The association of multiple non-caseating granulomata and a positive Kviem test is normally considered to be indicative of a diagnosis of sarcoidosis. However, although depressed cell-mediated immunity is commonly described, it is extremely rare to find a humoral immune paresis. A patient is reported who had multiple granulomata, depressed cellular and humoral immunity and a positive Kveim test.

A 13-year-old boy with severe, active granulomatous ileocolitis presented with bilateral conjunctival nodules and infiltrates, iritis, and uniocular corneal infiltrates. Gastrointestinal biopsy revealed deep focal ulcers of the bowel wall with adjacent granulomas. Conjunctival biopsy disclosed non-caseating granulomatous inflammation.
