Hemifacial spasm consists of involuntary contractions of the facial muscles, usually caused by vascular compression of cranial nerve VII , near its exit zone from the brainstem [17]. Arteriovenous malformations (AVMs) are a rare cause of this condition [2, 3]. We report a case of hemifacial spasm caused by an extensive cerebellar AVM, which was successfully treated by intravascular embolization .

Cerebrospinal fluid from patients with subarachnoid haemorrhage and cerebral arterial spasm contracted the isolated basilar artery and potentiated contractions produced by 5-hydroxytryptamine, noradrenaline, and six prostaglandins. The unidentified substance in cerebrospinal fluid probably plays a role in the aetiology of cerebral arterial spasm.

The correlation between obstructive sleep apnea (OSA) and hemifacial spasm has never been reported in the literature. Here, we report a case of OSA-induced hypertension with intractable hemifacial spasm in which both conditions improved after continuous positive airway pressure treatment.

We used neurointerventional techniques to conduct a functional investigation of the artery responsible for hemifacial spasm in a 48-year-old woman. Insertion of a microcatheter into the posterior inferior cerebellar artery stopped the hemifacial spasm immediately and completely. The artery was verified intraoperatively as the vessel compressing the root exit zone of the facial nerve.
