心不全患者において，ジギタリスは洞調律では強心作用を，心房細動（atrial fibrillation：AF）ではレートコントロールを目的に使用されることが多い．いずれも血中濃度の有効治療域は狭く，薬物治療モニタリングが必須である．近年，心不全を合併したAFのレートコントロールに対する長期的なジギタリスの有益性は低く考えられている．一方，カテーテルアブレーション等の有効性は多数報告されており，心不全を合併したAF管理の最新の知見も紹介する．
