The effect of Rectal Retractor, a rectal protector on rectal dose in radical prostate radiotherapy
نویسندگان
چکیده مقاله:
Introduction: The close proximity of the rectum tissue to the prostate gland results in rectal toxicity during prostate radiotherapy. Rectal Retractor (RR) is a rectal displacement technique that inserts into the rectum, as a result, the lateral and dorsal part of the rectal wall push away from high dose areas. The aim of this study was to investigate the dosimetric consequences and rectal tissue sparing effect of RR during prostate three-dimensional radiotherapy (3DCRT). Materials and Methods: In 15 patients with localized prostate cancer, two planning CT- scans were carried out: without and with RR. The prostate and seminal vesicles plus 5 mm margin were considered as planning target volume (PTV). All patients were treated using 3DCRT with prescription dose of 80 Gy. The inter-fraction setup errors were corrected by electronic portal imaging device (EPID) and fiducial gold markers. Dose volume histograms of the rectum were compared in plan without and with RR. Results: All patients well tolerated RR during first 20-25 fractions. Using RR, mean dose to the rectum was 39 cGy (P <0.001) lower per fraction. Comparison between plan without and with RR indicated that RR resulted in a mean reduction of 57.3%, 63.8%, 71.9% and 72.8% in rectal V50Gy, V60Gy, V70Gy and V75Gy, respectively (P <0.001). Conclusion: The retraction of the rectum is associated with a significant lower dose to the rectum. This may lead to reduction of the rectal toxicity during prostate radiotherapy.

Prostate IMRT: Two-dimensional model of rectal NTCP employing the variability of rectal motion and rectum wall thickness
Background: In order to improve the evaluation of possible rectal toxicity based on the rectal normal tissue complication probability (NTCP), we consider the fractional dependence of the NTCP on the wall thickness (tW) and rectal displacement (RM). Materials and Methods: The two-dimensional NTCP model (NTCP2D) was developed using radiotherapy plans of ten randomly selected patients with prostat...
متن کاملRectal cancer: a review
Rectal cancer is the second most common cancer in large intestine. The prevalence and the number of young patients diagnosed with rectal cancer have made it as one of the major health problems in the world. With regard to the improved access to and use of modern screening tools, a number of new cases are diagnosed each year. Considering the location of the rectum and its adjacent organs, ma...
متن کاملRectal wall dose estimation in intracavitary brachytherapy: A preliminary comparison of an in-house rectal wire versus ICRU 38 recommendations
Background: In intracavitary brachytherapy for gynecological cancers, various techniques are used to locate the anterior rectal wall nearest to the sources but there is no consensus on the best method to do so. This study aimed to compare a technique used routinely in some centers that employs a wire marker to locate the position of the maximum rectal dose point, versus the method recommended b...
متن کاملRectal dose-volume differences using proton radiotherapy and a rectal balloon or water alone for the treatment of prostate cancer.
PURPOSE To describe dose-volume values with the use of water alone vs. a rectal balloon (RB) for the treatment of prostate cancer with proton therapy. MATERIALS AND METHODS We analyzed 30 proton plans for 15 patients who underwent CT and MRI scans with an RB or water alone. Simulation was performed with a modified MRI endorectal coil and an RB with 100 mL of water or water alone. Doses of 78-...
متن کاملRectal protection in prostate stereotactic radiotherapy: a retrospective exploratory analysis of two rectal displacement devices
INTRODUCTION High rectal doses are associated with increased toxicity. A rectal displacement device (RDD) reduces rectal dose in prostate stereotactic body radiation therapy (SBRT). This study investigates any dosimetric difference between two methods of rectal displacement (Rectafix and SpaceOAR) for prostate SBRT. METHODS Rectal dosimetry of 45 men who received SBRT within the PROMETHEUS tr...
متن کاملDoes setup on rectal wall improve rectal cancer boost radiotherapy?
BACKGROUND Rectal cancer patients that show a pathological complete response (pCR) after neo-adjuvant chemo-radiotherapy, have better prognosis. To increase pCR rates several studies escalate the tumor irradiation dose. However, due to lacking tumor contrast on online imaging techniques, no direct tumor setup can be performed and large boost margins are needed to ensure tumor coverage. The purp...
