The Comparison of deferoxamine, deferiprone and deferasirox iron chelators in decreasing serum ferritin level in patients with thalassemia major in Yasuj in year 2018

مقدمه: در تالاسمی ماژور به دلیل لیز بیش از حد سلولی بیماران دچار اضافه بار آهن خواهند شد. برای جلوگیری از عوارض اضافه بار آهن در بدن از آهن زداها استفاده می شود. آهن زداهای رایج مورد استفاده دفروکسامین، دفریپرون و دفراسیروکس می باشند. این مطالعه قصد دارد علاوه بر بررسی سطح فریتین سرم به مقایسه بین اثرات آهن زدا ها در کاهش سطح فریتین سرم در بیماران مبتلا به تالاسمی ماژور شهر یاسوج که در طی یکسال گذشته آهن زدا مصرف می کردند بپردازد. مواد و روش ها: این مطالعه مقایسه ای مقطعی است که بروی120 بیمار مراجعه کننده به بخش کولیز بیمارستان شهید بهشتی یاسوج در سال 1397 انجام پذیرفت. پس از ثبت اطلاعات مربوط به سن، جنس و نوع آهن زدای مصرفی نمونه خون جهت سنجش فریتین سرم از بیماران گرفته شد. نتایج: پس از حذف 114 بیمار در مطالعه وارد شدند. رنج سنی بیماران 3-38 سال و میانگین سنی بیماران 03/8 ± 20.56 سال بود. 53(46.5%) بیمار مذکر و 61 (53.5%) بیمار مونث بودند. میانگین سطح فریتین کل (ng/mL)3438±2872 بود. 14 نفر از آهن زدای دسفرال، 62 بیمار دفریپرون و 38 بیمار دفراسیروکس استفاده می کردند. بین آهن زدا ها از جهت کاهش سطح فریتین تفاوت معناداری وجود نداشت(P>0.05). از جهت کاهش سطح فریتین بین دو جنس ارتباط معناداری دیده نشد. (P>0.05). نتیجه گیری: بین آهن زداها از جهت کاهش سطح فریتین تفاوتی وجود ندارد. میانگین سطح فریتین در بیماران تالاسمی شهر یاسوج از حد نرمال بالاتر می باشد و نیاز به آموزش بیشتر بیماران در کنترل سطح فریتین می باشد تا عوارض ناشی از اضافه بار آهن کاهش یابد.
