Modified Internal Fixation Technique for Acromio-Clavicular (AC) joint dislocation: The “Hidden Knot Technique”
نویسندگان
چکیده مقاله:
Acromioclavicular (AC) joint injuries are common and often seen in contact athletes, resulting from a fall on the shouldertip with adducted arm. This joint is stabilized by both static and dynamic structures including the coracoclavicular (CC)ligament. Most reconstruction techniques focus on CC ligament augmentation as the primary stabilizer of the AC joint. The best surgical technique for some AC joint dislocations is still controversial. In this study, we explained a modificationof the CC ligament reconstruction technique described by Wellmann. The method is based on minimally invasive CCligament augmentation with a flip button/polydioxanone (PDS) repair, typically used for extracortical ACL graft fixation.Patients commonly complain that heavy sutures under the skin in subcutaneous tissue irritate the skin and sometimesrequire reoperation for suture removal. We present an augmentation technique that resolves this issue by changing thesuture knot location to the sub-clavicular position.

Background: The treatment of AC dislocation is based upon the classification scheme, Type I and II injuries can be treated symptomatically, The treatment of type III injuries remains somewhat controversial, While operative intervention is often reserved for types IV, V and VI. Although there are more than 60 surgical techniques reported for repair and reconstruction of Acromio-clavicular and Co...

(‘omplete dislocation of the acromio-clavicular joint is not a common injury. A review of the literature fails to disclose a carefully followed series numbering more than twenty-five identically treated cases. Treatment is indeed varied, and end-results uncertain. In the past four years we have been fortunate in encountering twenty-seven cases of complete acromio-clavicular dislocation (Fig. 1)...

Reconstruction of displaced acromio-clavicular joint dislocations using a triple suture-cerclage: description of a safe and efficient surgical technique
UNLABELLED PURPOSE In this retrospective study we investigated the clinical and radiological outcome after operative treatment of acute Rockwood III-V injuries of the AC-joint using two acromioclavicular (AC) cerclages and one coracoclavicular (CC) cerclage with resorbable sutures. METHODS Between 2007 and 2009 a total of 39 patients fit the inclusion criteria after operative treatment of ...

Background: The acromion clavicular joint dislocations are common injuries of the shoulder. The severity is dependentupon the degree of ligamentous injury. Surgical treatment is typically performed in higher grade acromioclavicularseparation with several static and dynamic operative procedures with or without primary ligament replacement. Methods: 47 patients with acute Rockwood type III, IV, an...
