Major Leg Wound Complications After Saphenous Vein Harvest for Coronary Revascularization

چکیده مقاله:
Background: Major leg wound complications after coronary artery bypass graft procedures are infrequent and few are reported in the litrature. We present our experience in treating 30 patients with major leg wound complications after coronary revascularization procedures. Materials and Methods: A retrospective review of 2100 bypass procedures with saphenous veingraft performed over a 5-years period was conducted.15 potential risk factors for those who developed Major leg wound complications were analyzed and compared with the entire cohort of patients undergoing similar bypass procedures during the same period. Results: lower extremity wound complications occurred in 30 patients (1.4%),19 of whom required additional surgical interventions. There were 6 wound debridments 2 skin grafts, 1angioplasty, 7fasciotomies,1embolectomy and fasciotomy 2 free tissue transfer. Of 14 variables evaluated by multivriate analysis, cardiopulmonary bypass time, duration of surgery and postoperative intraaortic ballon pump use were identified as significant independent predictors of major leg wound complications (p<0.05). Conclusion: The causes of major leg wound complications after saphenous vein harvest for coronary artery bypass graft procedures are multifactorial. To minimize these complications,we recommend vascular evaluations before saphenous vein harvest, attention to proper surgical tecnique, and careful harvest site selection.

Review of efforts to decrease costly leg wound complications in the medicare population following coronary revascularization.
BACKGROUND Current trends show that patients referred for coronary artery bypass grafting (CABG) are significantly older, sicker, and at higher risk for complications than ever before. Eliminating leg wound complications would significantly benefit these patients and reduce the consumption of health care time and dollars. Endoscopic vein harvesting (EVH) decreases the risk of wound complication...

BACKGROUND Endoscopic saphenous vein harvest (EVH) decreases leg wound infections and improves cosmesis after coronary artery bypass grafting (CABG). Recent data, however, suggest that EVH may be associated with reduced graft patency rates. The objective of this study is to assess the effect of EVH on short-term and midterm outcomes after CABG. METHODS Data were prospectively collected on all...

AIMS The aim of this study was to compare coronary artery bypass grafting (CABG) using either endoscopic (EVH) or open harvest (OVH). Leg-related morbidity and histological comparison of the veins were the outcome measures. METHODS One hundred consecutive patients scheduled for isolated CABG were randomly divided into two goups: an EVH and OVH group. Perioperative data were recorded. Patients...

The degree of cutaneous sensory deficit in the leg was assessed after removal of the long saphenous vein in 50 consecutive patients undergoing coronary artery bypass vein grafts randomly assigned subcutanous sutures or a single layer of sutures. Removal of the vein and repair of the leg incision were done by the same team of surgeons. In group 1 (25 patients) the leg incision was repaired with ...

Among the first 400 patients undergoing saphenous vein coronary artery bypass at Stanford Medical Center, 67 (18%) met criteria for "preinfarction angina". Angina was alleviated and possible myocardial infarction was prevented utilizing these surgical technics. The overall operative mortality rate in the preinfarction angina patients was 10.4%, which contrasted strikingly with a 1.5% operative ...
