Comparison of the right internal thoracic artery and radial artery as a second arterial conduit in ‘Y’ composite fashion in patients undergoing coronary artery bypass grafting using total arterial revascularization

چکیده مقاله:
Introduction: The use of two arterial conduits for CABG is rapidly increasing. The second arterial conduit to LITA is usually RITA or radial artery. We sought to compare outcomes when either RITA or radial artery is exclusively used as a Y composite graft to LITA for total arterial revascularization. Material and methods: We retrospectively analyzed 231 patients who underwent CABG in the period from 2010 to 2014. RITA was used in 178 patients (RITA group) and radial artery was used in 53 patients (radial group). Results: Radial was used more frequently in female patients and in diabetic patients. Radial group had comparable number of distal anastomoses and lesser operative time to RTIA group. Early postoperative outcomes (low cardiac output syndrome, post-operative myocardial infarction, use of intra-aortic balloon pump, post-operative stroke, re-explorations, incidence of deep sternal wound infection and death) were all comparable in both the groups. Late deaths and need for repeat revascularization were also similar in both the groups for up to 4 years after surgery. Only the incidence of major acute cardiac and cerebrovascular events (MACCE) was more in the radial group. Conclusion: Radial artery has comparable short- and mid-term outcomes to RITA when used as a second arterial conduit in CABG. Its use should be especially considered in diabetic patients when DSWI is a concern.

Total arterial off-pump coronary revascularization with only internal thoracic artery and composite radial artery grafts.
OBJECTIVE Total arterial off-pump coronary artery bypass (OPCAB) grafting with only internal thoracic artery (ITA) and composite radial artery (RA) grafts has been applied extensively to avoid cerebral complications and late vein graft failure. We evaluated the initial experience with this method by clinical and angiographic study. METHODS Between April 2000 and May 2002, 257 patients underwe...

Coronary artery bypass graft (CABG) operations are one of the most commonly performed sur‐ gical procedures, with a worldwide prevalence of over 800,000 annually and more than 350,000 operations being performed in the United States each year [1]. The use of the left internal mam‐ mary artery (LIMA) is widely considered to be the gold standard for conventional CABG opera‐ tions. Its use has been...

OBJECTIVE We sought to compare early clinical outcomes in patients receiving a right internal thoracic artery or a radial artery as the second arterial graft in myocardial revascularization. METHODS We retrospectively studied 58 consecutive patients who underwent coronary artery bypass surgery and received both a left internal thoracic artery graft and either a right internal thoracic artery ...
متن کاملElongation of right internal thoracic artery with radial artery for redo total arterial revascularization in patients with open left internal thoracic artery to left anterial descending artery graft.
Total arterial revascularization in patients with three-vessel disease cannot usually be achieved by bilateral internal thoracic artery grafts alone due to limited length. In this report we describe a technique by which the right internal thoracic artery can be elongated with radial artery for sequential grafting in redo total arterial revascularization procedure.
متن کاملShould bilateral internal thoracic artery grafting be used in elderly patients undergoing coronary artery bypass grafting?
BACKGROUND Although bilateral internal thoracic artery grafting is associated with improved survival, the use of this technique in the elderly is controversial because of their increased surgical risk and shorter life expectancy. The purpose of this study was to evaluate the effect of age on outcome of patients undergoing bilateral internal thoracic artery grafting. METHODS AND RESULTS Betwee...
متن کاملThe left internal thoracic artery and radial artery composite graft in off-pump coronary artery bypass grafting.
Arterial multivessel bypass grafting without extra corporeal circulation and manipulation of the ascending aorta should be a good surgical option for the treatment of coronary artery disease. An internal thoracic artery (ITA)-radial artery (RA) composite graft was used for this purpose. Between July 2000 and October 2001, we employed the LITA-RA composite graft for off-pump coronary artery bypa...
