Closed K-wire Fixation for the Treatment of Perilunate Dislocation and Trans-Scaphoid Perilunate Fracture Dislocations without Ligamentous Repair: Short Term Follow-Up

چکیده مقاله:
Background: The purpose of this study was to evaluate the treatment outcomes of perilunate dislocations (PLDs) andtransscaphoid perilunate fracture dislocations (TSPLDs) treated with operative volar approach without ligament repairor reconstruction. Methods: A total of seven patients (10 hands) were studied during 2017-2018. We fixed scaphoid fractures thorougha volar approach and 3 k-wires were inserted to stabilize the scaphoid to the adjacent bones. We evaluated patients’range of motion by VAS and grip strength, sensitivity assessment, and radiographs. Mayo and DASH scores were alsorecorded for wrist evaluation. Results: All fractures united well. The mean Mayo wrist score was 81.5. Five patients scored good and excellentresults (90-100). Excellent DASH scores (˂20) were observed in 4 patients and there was no poor DASH score (˃50).Conclusion: Treatment of PLDs and TSPLDs with k-wire and screw fixation using a volar approach and without anyligament repair or reconstruction results in minimal manipulation and has favorable short-term outcomes.Level of evidence: II

Trans-Scaphoid Perilunate Fracture-Dislocation and Isolated Perilunate Dislocations; Surgical Versus Non Surgical Treatment
Background: Trans-scaphoid perilunate fracture-dislocation and perilunate dislocations are among uncommon injuries, most commonly seen in young patients due to high energy trauma. The treatment can be achieved either surgically by open reduc tion and internal fixation or closed reduction and casting. Methods: To compare surgical versus non-operative results of treatment of trans-scaphoid pe...

متن کاملBilateral Trans-Scaphoid Perilunate Fracture Dislocation
The wrist represents a complex anatomic region in the human extremity and a highly functional and intricate structural joint. Most perilunate injuries result from a staged disruption of the ligamentous structures. Forces may be directed through the carpal bones themselves resulting in a fracture. Early anatomic reduction of any carpal malalignment is of the utmost importance. Unfortunately, eve...

Rowan CH, Downing ND. BMJ Case Rep 2017. doi:10.1136/bcr-2017-221041 Description Perilunate dislocations are uncommon high-energy injuries which are missed in approximately 25% of cases on initial presentation. We present the X-rays of a 25-year-old man who fell from a height of approximately 2 metres onto an extended wrist. Plain radiographs (figure 1) show classical signs of a trans-scaphoid ...
متن کاملPercutaneous Herbert screw osteosynthesis in trans-scaphoid perilunate fracture-dislocations.
UNLABELLED Trans-scaphoid perilunate fracture-dislocation is the most common variety of fracture-dislocations of the wrist. Because of the carpal scaphoid fracture association, post-reduction stability is poor, often requiring surgery. Of the several surgical techniques used over time, percutaneous screw fixation for scaphoid fracture is currently preferred. MATERIAL AND METHODS Six cases of ...
