چکیده مقاله:

Background: The role of wound drainage after total knee arthroplasty is still considered controversial as althoughclosed drainage systems have been believed to be effective in decreasing the post-operative complications, they couldalso facilitate the bleeding and increase the rate of transfusion and infection. We have conducted the current study tocompare the outcomes superficial subcutaneous, one deep, and two deep drain techniques after total knee arthroplasty. Methods: Between 2014 and 2015 sixty consecutive patients were prospectively selected and underwent primarytotal knee arthroplasty. Patients randomized to receive one superficial, one deep and two deep drains at the end ofoperation. Tourniquet was used and opened at the end of the surgery after dressing. Patients were studied for volumeof blood loss, hemoglobin drop, number of transfusion, and any complications. Knee range of motion and diameterwere measured and compared with contralateral side in all cases at the end of the third day. Results: There was no statistical difference regarding red blood cell volume loss, Hb drop, and transfusion rate betweengroups. Patients in one superficial group had the most sever post-operative ecchymosis. Knee flexion and swellingwere the same in all groups. Patients in one superficial drain group had the worst VAS for the pain. Need for earlyblood transfusion was significantly higher in two deep drain group. In one deep drain group returned back to operatingroom for sever hemarthrosis and wound dehiscence was occurred in a patient. One patient in one deep group had alsodeveloped mild thrombo-emboli.Conclusion: Regarding the blood volume loss after total knee arthroplasty there is no difference betweensuperficial drainage and even more effective intra-articular techniques. Outcome and complication rates are thesame.

Seasonal Influence on Postoperative Complications after Total Knee Arthroplasty

Purpose This study is aimed at investigating whether inpatient complications and surgical site infections (SSIs) occurred more commonly in patients undergoing total knee arthroplasty (TKA) during the summer season. Materials and Methods A total of 725 patients who underwent unilateral or bilateral TKA were included in this study. A total of 241 patients (33.2%) underwent TKA between May and A...

Knee Fusion or Above-The-Knee Amputation after Failed Two-Stage Reimplantation Total Knee Arthroplasty

Prosthetic joint infection (PJI) is a serious complication of total knee arthroplasty (TKA). Control of infection after a failed two-stage TKA is not always possible, and the resolution of infection may require an above-knee amputation (AKA) or a the-knee (KF). The purpose of this review is to determine which treatment method (AKA or KF) yields better function and ambulatory status for patients...

Comparison between Patellar Resurfacing and Retention in Total Knee Arthroplasty Regarding the Postoperative Satisfaction of Patients and Patellar Crepitus

Background: Patellar crepitus after total knee arthroplasty (TKA) is not uncommon. The choice between patellarresurfacing or retention in TKA has remained controversial. Therefore, this randomized controlled trial aimed to evaluatethe impact of patellar resurfacing on the incidence of patellar crepitus. In addition, we compared the clinical outcomesand satisfaction between the...

Postoperative analgesia after total knee arthroplasty.

We read with great interest the article of Wang et al.1 concerning the use of continuous local anesthetic infusion via catheters placed with ultrasound guided and nerve stimulator beside femoral nerve for postoperative analgesia after total knee arthroplasty (TKA). We congratulate them on the presentation of the article. However, we would like to add some comments. We agree with author that reh...

The Role of Drainage After Total Knee Arthroplasty

Total knee arthroplasty (TKA) is associated with significant postoperative blood loss for which blood transfusion might be necessary. The role of wound drainage is controversial. The use of drainage was believed to be effective in decreasing hematoma formation (Drinkwater and Neil 1995; Holt et al. 1997; Martin et al. 2004), which has been theoretically thought to decrease postoperative pain, s...

Complications of Total Knee Arthroplasty

Total knee arthroplasty (TKA) is an excellent surgical procedure that can make severely damaged knees painless and stable, but it can be associated with many complications. Once complications develop, their management is not necessarily easy. The major complications include loosening, wear and breakage of the prosthetic components, patellar problems, fracture in the vicinity of the prosthesis, ...
