Subacute Inflammatory Arthritis Following Intravascular Coagulation Syndrome

Abstract:

Un nourisson de six mois a ete hospitalise dans le .,ervice de pediatrie du C.H.U. de Logmandolhe-Adham pour: deshydratation, etat septique, acidose metabolique, troubles circulatoires des membres avec gangrene de la main droite.  Le syndrome de la coagulation intra-vasculaire disse­minee (CID) a ete rapidernent confirme par l'etude de la erase sanguine. Soumis a un traitement par: solutes 6lectrolytiques, bicarbonates, antibiotiques, heparine et carticoides, l'enfant a gueri en une dizaine de jours. La main droite gangrcnee a ete par la suite amputee.  L'enfant a ete rehospitalise a l'age de huit mois en L'enfant a ete rehospitalise al'age de huit mois en des signes inflammatoires cliniques et biologiques locaux :t generaux.  La recherche du facteur rhumatoide etait negative dans le sang et fortement positive dans le liquide synovial. Celui-ci avait un aspect puriforme aseptique. La biopsie a mis en evidence une synovite inflammatoire subaigue. La guerison de J'arthrite a ete obtenue en f!ieux mois environ, l'effet du salicylate a ete spectaculaire. Revu un an plus tard, l'enfant ne presentait aucune atteinte articu­aire.  Cette oligo-arthrite inflammatoire d'allure rhumatoi­je observee au decours d'un syndrome de CID souleve une question prirnordiale: le role eventuel des thromboses va!-:culaires dans la gencse des lesions articulaires.
