Abstract:

Background To assess the usefulness of premature luteinization hormone (LH) surge prevention in an intrauterine insemination (IUI) cycle by GnRH antagonist administration MaterialsAndMethods Sixty patients with unexplained or mild male infertility or minimal to mild endometriosis were enrolled in this prospective randomized controlled trial. There were twenty patients in group A (with GnRH antagonist) and 40 patients in group B (without GnRH antagonist). In all of the participants, clomiphene citrate and human menopausal gonadotropin (CC+HMG) were used for ovarian stimulation. When at least one follicle with ≥ 16 mm diameter was seen, LH surge was checked by a urinary LH kit. In patients with negative results, human chorionic gonadotropin was continued in both groups, but in group A 0.25 mg Ganirelix SQ was administered for two days, ,then in both groups human chorionic gonadotropin (HCG) was injected on the third day and IUI was done 36-40 hours later. Ongoing pregnancy was the primary outcome. Results Baseline characters and clinical parameters were similar in both groups with the exception of ≥14 mm follicles which were higher in group A (p value= 0.003). The pregnancy rate in both groups was not significantly different, although it was higher in group B (10% in group A and 15% in group B). Conclusion At least in CC+HMG stimulated cycles for IUI, the occurrence of premature LH surge could have a useful rule and GnRH antagonist administration could be an inappropriate intervention.

Evaluation of Role of GnRH Antagonist in Intrauterine Insemination (IUI) Cycles with Mild Ovarian Hyperstimulation (MOH): A Prospective Randomised Study.

AIMS AND OBJECTIVE To evaluate the role of GnRH antagonist in prevention of premature LH surge and increasing pregnancy rates in IUI cycle with mild ovarian hyperstimulation (MOH). STUDY DESIGN Prospective parallel, randomised controlled study. MATERIAL AND METHODS Couples diagnosed with unexplained, male factor subfertility and with one or both tubes patent were randomised to receive eithe...

Timing ovulation for intrauterine insemination with a GnRH antagonist.

BACKGROUND We aimed to assess the efficacy of a GnRH antagonist in intrauterine insemination (IUI) cycles to increase number of mature ovulatory follicles and pregnancy rates. METHODS Prospective randomized study. Women (18-38 years old) with primary/secondary infertility were included. Eighty-two patients were randomly assigned to controlled ovarian stimulation (COS) consisting of rFSH + GnR...

Antagonist use in intrauterine insemination (IUI) cycles İntrauterin inseminasyon sikluslarında antagonist kullanımı

Özet Intrauterine insemination is the first method of treatment for many causes of infertility, mainly unexplained infertility, male subfertility, and ovulatory dysfunction.Despite its popularity, the effectiveness of IUI treatment is not consistent, and the role of IUI treatment in practice protocols has not been clarified. The success of IUI depends on a number of parameters linked both to the p...

Reply: Effect of GnRH antagonists in FSH mildly stimulated intrauterine insemination cycles: a multicentre randomized trial

Sir, We would like first to thank Drs. Niraj Mahajan, Kshitija Mahajan and Rajani Soni for their interest in our paper (Crosignani et al., 2007). In their brief comment, these authors have emphasized several important albeit still unclear aspects of controlled ovarian hyperstimulation (COH) and intrauterine insemination (IUI). Does double insemination provide any benefit? What is the most suita...
