I-46: Repeat Transvaginal Ultrasound-Guided Ovarian Interstitial Laser Treatment Improved the Anovulatory Status in Women with Polycystic Ovarian Syndrome
Authors
Abstract:
Background: To assess the effectiveness of repeated transvaginal ultrasound-guided ovarian interstitial laser treatment in the management of anovulatory patients with polycystic ovary syndrome (PCOS). Materials and Methods: A retrospective study was performed in our department on 27 anovulatory PCOS women who failed to respond to the first ultrasound-guided transvaginal ovarian interstitial laser treatment. Each woman received a repeat procedure and postoperative rates of ovulation and pregnancy were monitored. Results: Of 27 patients, 18 (66.67%) ovulated spontaneously following the repeated ultrasound-guided transvaginal ovarian interstitial laser treatment Ten women became pregnant within six postoperative months and a cumulative pregnancy rate of 37.04% was achieved. There were no significant operative complications. Conclusion: Repeated ultrasound-guided transvaginal ovarian interstitial laser treatment is an effective and safe management in anovulatory PCOS patients who fail to respond or have a transient response to the first transvaginal ovarian interstitial laser treatment.

Background: In 2005, we designed and evaluated the effectiveness of the transvaginal ultrasound-guided ovarian interstitial laser treatment in twenty-three anovulatory women with clomifene citrate (CC)-resistant polycystic ovary syndrome (PCOS) as a new method of ovulation induction in infertile PCOS women, with a more than 80% ovulation rate and 36% pregnancy rate during six postoperative mont...

Ultrasonography is the primary imaging tool for a patient considered to have an ovarian cyst. On a sonogram, simple ovarian cysts have a uniformly thin, rounded wall and a unilocular appearance that is either hypoechoic or anechoic. They usually measure 2.5-15 cm in diameter, and posterior acoustic enhancement (a hyperechoic area) may be visible deep to the fluid-filled cyst. Most commonly, the...

Results: Follow-up data, which were available for 181 patients, showed a spontaneous ovulation rate of 70.1%, cumulative ovulation rate of 98.3%, and pregnancy rate of 84.5%. Women who conceived following surgery were obese, had higher pre-operative luteinizing hormone (LH) levels, were younger and were more likely to have typical ultrasonographic features of polycystic ovarian disease. Logisti...
