I-41: Imaging Strategy for Diagnosis of Adnexal Masses Including US,CT Scan and MRI
author
Abstract:
Adnexal masses are spectrum of diseases from benign non neoplastic and neoplastic masses to malignant neoplasms. Endovaginal ultrasonography (US) is the fisrt and invaluable modality for assessment of adnexal masses because it is available and has a high negative predictive value. Important morphologic features are solid (vascularized) tissue, vascular and thick septations, and papillary projections on gray scale and color doppler US. Spectral Doppler analysis parameters (RI and PI and S/D ratio) correlate well with malignancy but generally add little information. CT scan can help in staging malignant ovarian cancers but some indeterminate adnexal masses need more detailed imaging by Magnetic resonance (MR) imaging. Using MR imaging for diagnosis of adnexal masses includes morphologic characteristics and signal intensity on T1- and T2-weighted images. Cystic masses are probably benign tumors, whereas complex masses are strongly associated with malignancy. Cysts, mature cystic teratomas, leiomyomas, endometriomas and fibromas, can be accurately determined on the basis of T1, T2 and fat-saturated T1-weighted sequences. New MRI protocols such as dynamic enhanced MRI (DCE-MRI) and MRspectroscopy can be used as adjuant to simple MRI to increase diagnostic accuracy for characterizing adnexal masses.

I-48: Approach to Adnexal Masses
Most ovarian masses are benign conditions such as non neaplastic cystic, but on the other hand , ovarian cancer, is the third most common gynecological malignancy which has the highest mortality rate , as a result of late diagnosis. As there are few clinical symptoms, most patients at the Abstracts of the 12th Royan International Congress on Reproductive Biomedicine 2 3 International Journal of...
full textValue of MRI in Characterizing Adnexal Masses.
Magnetic resonance imaging (MRI) increases the specificity of imaging evaluation for adnexal masses, especially when they are indeterminate on ultrasound, very large or when further imaging is required to establish tissue characteristics. This article is a pictorial essay describing the value of MRI in characterizing adnexal masses.

Adnexal masses represent a spectrum of conditions from gynecologic and nongynecologic sources. They may be benign or malignant. The initial detection and evaluation of an adnexal mass requires a high index of suspicion, a thorough history and physical examination, and careful attention to subtle historical clues. Timely, appropriate laboratory and radiographic studies are required. The most com...

PURPOSE To assess clinical factors, histopathologic diagnoses, operative time and differences in complication rates between women undergoing laparoscopy or laparotomy to diagnose and treat an adnexal mass and their association with laparoscopy failure. METHODS In this prospective study, 210 women were invited to participate and 133 of them were included. Eighty-eight women underwent laparotom...

Compared with ultrasound, MRI has limited indications in evaluation of Female Infertility. Laparoscopy is the gold standard for the diagnosis of pelvic endometriosis. MRI is helpful in determining the extent of deep infiltrating endometriosis, especially when laparoscopic inspection is limited by adhesions. MRI allows for much more anatomic detail as well as better measurements of uterine fibro...
full textAdded Value of Assessing Adnexal Masses with Advanced MRI Techniques
This review will present the added value of perfusion and diffusion MR sequences to characterize adnexal masses. These two functional MR techniques are readily available in routine clinical practice. We will describe the acquisition parameters and a method of analysis to optimize their added value compared with conventional images. We will then propose a model of interpretation that combines th...
