Abstract:

The learning objectives of this presentation are to understand the dynamics involved in the process of ET, evaluate the evidence for/against common practices and techniques and develop a standardized ET process in view of supporting evidence. Gametes and embryos are handled with extreme care at every step of the Laboratory process. ET is the least sophisticated step of the in vitro Fertilization process, making it the most vulnerable. The working hypothesis is that designing a Standardized Embryo Transfer Approach is expected to maintain higher pregnancy rates and lower adverse effects. Experimental findings suggest that positioning of the patient while keeping the uterine fundus at the highest point in the sagittal cross-section above the horizon, placement of the catheter tip at mid cavity about 2.0 cm from the fundus and delivery of the load over a course of 10 s or more, may be associated with the desired outcome. Evidence supports: a definite proof of benefit for the use of soft embryo transfer catheters and ultrasound guidance; a limited proof of benefit for mid-uterine position of catheter tip, acupuncture, use of hyaluronic acid , shortening of the loading discharging interval time of embryos and mechanical closure of the cervical canal following ET; and no proof of benefit for the use of mock transfer prior to ET, antibiotic administration, full bladder, removal or flushing of the cervical mucus, use of fibrin sealants and bed rest following ET.

Embryo metabolism: what does it really mean?

The study of early embryo metabolism has fascinated researchers in the field for nearly a century. Herein, we give a brief account of the general features of embryo metabolism and some consideration of the research performed to reach such conclusions. It is becoming increasingly obvious that metabolism informs many fate decisions and outcomes beyond ATP generation, such as DNA methylation, Reac...

Rewarding creativity: when does it really matter?

This study examined the possibility that the relation between extrinsic rewards (e.g., pay and recognition) and employee creativity varied as a function of two conditions: employee job complexity and employee cognitive style. Our results showed a positive relation between extrinsic rewards and creativity for employees with an adaptive cognitive style who worked on relatively simple jobs. We fou...

Wiring Watkins University: Does IT Really Matter?

This case describes the “wiring” of Watkins University (a fictional name for a real Midwestern university) between 1997 and 2003 as the university responded to competitive pressures in the higher education market. After describing the University and the competitive challenges it faced, the case takes the student into a strategy session between the organization’s CFO, CTO and Provost as they rev...

Why Is Saline So Acidic (and Does It Really Matter?)

Commercial 0.9% saline solution for infusion has a pH around 5.5. There are many reasons for this acidity, some of them still obscure. It is also true that infusion of normal saline can lead to metabolic acidaemia, yet the link between the acidity of saline solution and the acidaemia it can engender is not straightforward. This commentary draws together the known and putative sources of acidity...

Perioperative myocardial ischemia: pathophysiology and does it really matter.

To the Editor:—In an excellent review of myocardial protection during anesthesia, Warltier et al. mention the possibility of improving oxygen availability to the myocardium by a right shift of the oxyhemoglobin dissociation curve. The beneficial effects of b-blocking drugs also are reviewed. However, the correlation of the well-known fact that b-blockers shift the curve to the right is omitted....

Obesity in total hip arthroplasty—does it really matter?

BACKGROUND AND PURPOSE Discussion persists as to whether obesity negatively influences the outcome of hip arthroplasty. We performed a meta-analysis with the primary research question of whether obesity has a negative effect on short- and long-term outcome of total hip arthroplasty. METHODS We searched the literature and included studies comparing the outcome of hip arthroplasty in different ...
