Evaluation of Cinical and Radiological Results of Calcaneal Lengthening Osteotomy in Pediatric Idiopathic Flexible Flatfoot
Authors
Abstract:
Abstract: Background: Flexible idiopathic flatfoot is the most common form of flatfoot. First line treatments are parental reassurance and conservative measures. However, surgical treatment may be needed in some cases. A number of surgical techniques with varying results have been described in the literature. Here, we present our clinical and radiological outcomes of calcaneal lengthening osteotomy in pediatric idiopathic flexible flatfoot. Methods: Calcaneal lengthening osteotomy was performed in 20 patients, 30 feet, with idiopathic flexible flatfoot that were resistant to conservative treatment between 2007 and 2011. Patients were evaluated according to ACFAS universal evaluation scoring scale and radiographic indexes. Mean follow up was 23.1 +/_ 9.9 months. Results: The average age was 10.4 +/_ 0.9 years. Achilles tendon lengthening was performed in 28 feet. At final follow up, ACFAS score had improved significantly (P value < 0.0001) compared to pre-operative score (37 versus 88). The radiographic parameters also showed significant improvement after surgery. Distal segment displacement and hardware irritation as complications were observed in 2 and 3 cases respectively without long term clinical impact. Conclusion: Calcaneal lengthening osteotomy is an appropriate and safe operation in symptomatic idiopathic flexible flat foot that is resistant to conservative treatment.

The Evans calcaneal osteotomy for correction of flexible flatfoot syndrome.
The Evans calcaneal osteotomy is used to correct the flexible flatfoot deformity. The procedure restores functional integrity to the medial longitudinal arch and reestablishes the locking mechanism of the midtarsal joint complex. A preliminary analysis of 36 cases (50 feet) performed at Atlanta Hospital and Medical Center during the past 3 years has yielded favorable results.

BACKGROUND The timing and strategy of treatment for flatfoot still remain controversial. It is a difficult problem when facing severe adolescent flexible flatfoot because a single procedure cannot realign flatfoot deformity effectively. METHODS We reviewed 13 adolescent flexible flatfoot patients who underwent double calcaneal osteotomy during May 2012 to June 2015. The mean age of patients w...

BACKGROUND The Evan's calcaneal lengthening osteotomy is a treatment method for spastic flatfoot deformity in patients with cerebral palsy that fail nonoperative measures. Autograft and allograft have been reported as potential graft choices. Bovine xenograft has been introduced as an alternative, but limited human data exists supporting its efficacy. This study compares the long-term results o...
