Does Adding Lidocaine to Intrathecal Bupivacaine Affect Hemodynamic Parameters during Hip Fracture Surgery?
Authors
Abstract:
Background: Hip fracture is one of the most common problems in elderly that needs surgical repair. As, the majority ofthese patients have chronic diseases, they are at increased risk of peri-operative mortality and morbidity. The purposeof this study was to evaluate spinal anesthesia with bupivacaine vs bupivacaine in combination with lidocaine in termsof hemodynamic changes in patients undergoing hip fracture surgery. Methods: This double-blind clinical trial was conducted on 292 patients undergoing surgery for hip fracture underspinal anesthesia. Patients were allocated into two groups of B (10 mg of hyperbaric 0.5% Bupivacaine) and BL (5 mghyperbaric Bupivacaine 0.5% plus 50 mg Lidocaine 5%). Sensory and motor block and hemodynamic changes wereconsecutively measured before spinal anesthesia (T0), immediately after spinal injection (T1), every 5 minutes for halfan hour (T2- T7), and at 45 minutes (T8) and 60 minutes (T9) after injection. Results: Patients in the two groups were homogeneous in demographic characteristics including age, sex, BMI, ASAClass, baseline blood pressure and heart rate. The onsets of sensory and motor blocks in group BL were faster thangroup B (P=0.0001). Also, the durations of sensory and motor blocks in group B were significantly longer than groupBL (P=0.0001). The BL group had a significantly lower systolic blood pressure in all periods (P

Does Venous Thromboembolism Affect Rehabilitation after Hip Fracture Surgery?
PURPOSE Venous thromboembolism (VTE), including deep vein thrombosis and pulmonary embolism, is a serious and life-threatening complication in elderly (older than 65 years) patients undergoing hip fracture surgery (HFS). However, few have reported on the influences of VTE on postoperative rehabilitation in these patients. This study was performed to determine whether VTE affects clinical outcom...
full textThe effects of Adding Meperidine to Heavy Intrathecal Lidocaine on Hemodynamic Changes and Blood Loss in Open Prostatectomy: A Randomized Double-Blind Clinical Trial
Background: Clinical investigations have reported several anesthetic properties of intrathecal injections of meperidine. The purpose of this study was to investigate the effect of adding meperidine to intrathecal heavy lidocaine on hemodynamic changes and blood loss in patients undergoing elective suprapubic open prostatectomy. Methods: In a randomized double-blind clinical trial, 77 pa...
full textThe duration of intrathecal bupivacaine mixed with lidocaine.
BACKGROUND Although spinal bupivacaine may have too long duration to be useful in the ambulatory setting, recent animal data suggest that lidocaine added to spinal bupivacaine may reduce the duration of bupivacaine spinal anesthesia. We explored whether lidocaine added to spinal bupivacaine could shorten the duration of bupivacaine spinal anesthesia in humans similarly to what has been reported...
full textDoes bone mineral density affect hip fracture severity?
The association between hip fracture and reduced bone mineral density is well documented, with reduced bone mineral density predisposing to fracture. However, it is unknown whether an association exists between the magnitude of bone density lost and the severity of the hip fracture sustained. One hundred forty-two patients (96 women, 46 men) with a mean age of 74 years (range, 49-92 years) who ...
full textDoes speed of intrathecal injection affect the distribution of 0.5% hyperbaric bupivacaine?
We have evaluated the influence of speed of intrathecal injection on lateral distribution of 0.5% hyperbaric bupivacaine. We studied 60 patients undergoing lower limb surgery who were placed in the lateral position with the operative side in the dependent position. After dural puncture (25-gauge Whitacre spinal needle), the needle aperture was turned towards the dependent side and 0.5% hyperbar...
full textComparison of the Effect of Intrathecal Bupivacaine with Intrathecal Ropivacaine on Hemodynamic of Patients under Elective Caesarean Section
Background and Objectives: The correct management of delivery and anesthesia hs important for maternal and fetal health outcome during cesarean section. The aim of this study was to compare the effect os spinal anesthesia with bupivacaine and ropivacaine on the hemodynamic parameters of pregnant women during cesarean section. Methods: This study was a randomized double blinded clinical trial o...
