Congenital Absence of Renal Artery: Introducing Two Patients

Abstract:

Les auteurs font une revue de la litterature a propos de deux cas de l'agenesie de l'artere renale, anomalie rare, dont les circonstances de decouverte varient selon les cas: infections recidiventes des voies urinaires, anomalie du Perinee, hypertension arterielle.  La premiere observation concerne une fillette a.gee de 7 ans, hospitalisee pour une insuffi­sance cardiaque severe avec hypertension arte­rielle resistant aux therapeuthiques usuelles. Le bilan radiologique a mis en evidence une age­nesie de l'artere renale, une hypoplasie avec ptose du rein gauche. L'uretere gauche etait court, dilate et malforme. Du cote oppose, i1 existait un reflux ureterovesical important. La nephrectomie totale gauche a ete suivie d'une baisse de la tension arterielle et l'insuffisance cardiaque a considerablement regresse.  La deuxieme observation concerne une fillette agee de 8 ans, hospifalisee pour une incontinence des matieres et de l'urine, atteinte d'une hypo­plasie perineale avec malformation anale. Lors de la cystographie, au moment de l'effort de miction, l'urographine opacifiait le rectum, mais le trajet de la fistule n'etait pas decelable, la nature de cette communication n'a pu etre pre­cisee, les parents ayant refuse !'intervention. Le rein droit, mue: sur l'UIV apparaissait hypo­plasique, pfose et malforme (pylome double), lors de la Cystographie retrograde, l'existence des malformations anorectales et perineales chez ce malude s'expliquent par l'etroite relation entre le developpement des ebauches embryo­naires rectales et anogenitales.
