مفهوم و محاسبه‌ی شاخص DALYs در برآورد بار بیماری‌ها

Abstract:

همزمان با تغییر در ترتیب و توالی بیماری‌ها که از آن به "انتقال اپیدمیولوژیک بیماری‌ها" نام برده می‌شود و با هدف همسان‌سازی و سنجش مشترک مرگ‌های زودرس و ناتوانی‌ها، استفاده از یک مبنای مشترک ضرورت دارد. از اواخر دهه‌ی 1940، پژوهشگران "زمان" را به عنوان معیار مشترک سنجش مرگ‌های زودرس و ناتوانی‌ها تعیین و بر مبنای آن شاخص DALYs را معرفی کرده‌اند. این شاخص که از آن در ارزیابی‌های اقتصادی و محاسبه‌ی هزینه‌ها و عملکرد‌های بهداشتی استفاده می‌شود، متشکل از دو عامل "سال‌هایی از عمر که با مرگ زودرس از بین می‌رود" و "سال‌هایی از عمر که همراه با نوعی ناتوانی با شدت و طول مدت خاص سپری می‌شود"، می‌باشد. سازمان جهانی بهداشت نیز در تدوین مقادیر رهنمودی خود برای کیفیت میکروبی، شیمیایی، و رادیواکتیویته‌ی آب آشامیدنی از این شاخص بهره گرفته است. در این مقاله ضمن ارایه‌ی مفاهیم پایه از DALYs، چگونگی محاسبه‌ی صحیح آن در برآورد هزینه‌ی موثر، در قالب یک مثال کاربردی بیان شده است.
