does tranexamic acid reduce blood loss in off-pump coronary artery bypass?
Authors
abstract
tranexamic acid is now used on a routine basis for on-pump coronary artery bypass grafting (cabg). we assessed the hemostatic effects of tranexamic acid to decrease bleeding tendency and transfusion requirements in patients undergoing off-pump coronary artery bypass surgery (opcab). a total of 66 patients were enrolled to elective opcab in a double-blind, prospective randomized study. of these, 33 patients received tranexamic acid (15 mg/kg before the infusion of heparin and 15 mg/kg after protamin infusion), and 33 patients received only saline. preoperative hematologic variables, postoperative bleeding and allogeneic transfusions were considered. d-dimer plasma levels were also evaluated to monitor the activation of fibrinolysis. postoperative bleeding was significantly lower in the tranexamic acid group compared with the control group (320 ± 38 ml vs. 480 ± 75 ml at 12 hour, p < 0.001). the tranexamic acid group had significantly lesser need for allogeneic blood products (0.46 units/patients vs. 0.94 units/patients, p < 0.001). they had also lower post-operative d-dimer plasma levels. no postoperative thrombotic complications were observed in either group. the defective hemostasis occurs even in the opcabg. tranexamic acid effectively reduces postoperative blood loss and the need for allogeneic blood products after opcab is decreased.

The effect of local application of tranexamic acid to reduce blood loss after off pump coronary artery bypass grafting (CABG)
Method Total 150 patients were enrolled in this double-blind, randomized, placebo-controlled, prospective clinical trial scheduled for primary isolated off pump coronary artery bypass grafting, divided in 2 groups (each group consisting of 100 patients). Patients with coagulopathies, renal failure, re-do surgery, or recent anti-platelet treatment were excluded. Tranexamic acid (TA) group (75 pa...
full textEffectiveness of Moderate Acute Normovolemic Hemodilution Combined with Tranexamic Acid on the Reduction of Allogenic Blood Transfusion in Patients Undergoing Off-pump Coronary Artery Bypass
Introduction: There are different approaches to reduce the amount of blood loss and allogenic transfusion in cardiac surgery. Regarding this, the present study aimed to evaluate the blood sparing effect of acute normovolemic hemidilution (ANH) combined with intrao-perative tranexamic acid in patients undergoing off-pump coronary artery bypass (OPCAB).Material and Methods: This study was conduct...
full textEmergency Conversion from Off Pump to Cardiopulmonary Bypass in Patients with Coronary Artery Bypass Graft Surgery
Introduction: As off pump Coronary artery bypass grafting surgery (CABG) is a developing method in coronary cardiac surgery, most surgeons are anxious about the danger of emergency crash conversion. In this observational study we tried to show the rate and outcome of conversion . Material and methods: In this descriptive study about 477 coronary off-pump CABG patients were operated in Ghaem hos...
full textOff-Pump Versus On-Pump Coronary Artery Bypass Graft Surgery, prospective cohort study
Introduction: Coronary artery bypass grafting is a surgical operation that bypasses atherosclerotic coronary vessels and restores blood flow to the ischemic zone of hearts. And two different conventional methods are ON-pump CABG, and Off-pump CABG. Altogether, the efficacy and safety of these two types of surgical procedures are still under debate. In this study, we aim to com...

INTRODUCTION To examine whether or not off-pump CABG (Coronary Artery Bypass Reduce) reduces the incidence of AF after cardiac surgery. METHODS The study was carried out in 939 consecutive coronary artery disease patients with sinus rhythm from which 383 patients underwent off-pump CABG, and 556 patients were operated through on-pump CABG. All patients were monitored postoperatively during in...

Cardiovascular disease is the leading cause of death in the United States and most developed countries. The majority of deaths are due to coronary artery disease. It is estimated that 419,000 coronary artery bypass grafting (CABG) procedures were performed in the United States in 2009 for the surgical treatment of coronary artery disease. Off-pump CABG (without the use of cardiopulmonary bypass...
