the diagnostic value of end-tidal carbon dioxide (etco2) and alveolar dead space (avds) in patients with suspected pulmonary thrombo-embolism (pte)
Authors
abstract
introduction: capnography, is an easy, fast and practical method which its application in the diagnosis of pulmonary thromboendarterectomy (pte) has recently been studied. this study aimed to assess the diagnostic value of end-tidal co2 (etco2) and the alveolar dead space (avds) in the diagnosis of patients suspected to pte who have been referred to the emergency department. materials and methods: this cross-sectional study was conducted during one year in the emergency department of ghaem hospital on patients with suspected pte who scored less than 4 for the wells’ criteria during the initial evaluation. after excluding other differential diagnoses, all patients underwent ct pulmonary angiography (ctpa) to confirm pte. following that, arterial blood gas sampling, etco2 and avds were requested for all the patients based on capnography. data analysis was performed using descriptive statistical tests in spss software version 11.5. the sensitivity, specificity, and positive and negative predictive values of avds and etco2 were measured based on (ctpa) results. results: the study was performed on 78 patients (mean age of 47.08± 15.6 years, 43 males/35 females) suspected to pte. according to the results of ctpa, 37 patients did not develop pte while 41 patients were with pte. there was no significant difference between the two groups in terms of age and gender (p=0.999), while a statistically significant difference was found between the mean values of etco2 and avds between the two groups (p<0.001). the best cut-off points for pte diagnosis were 0.17 (based on avds, with sensitivity and specificity of 78.0% and 56.8%, respectively), and 26.5 (based on etco2, with sensitivity and specificity of 67.6% and 75.6%, respectively). in addition, the negative predictive values for avds and etco2 were estimated as 70.0% and 71.43%, respectively. conclusion: according to the results of this study, capnography could be effective to promptly rule out pte in emergency situations. given its negative predictive value to rule out pte, etco2 is considered as the most valid criterion among capnography parameters.

Background: The aim of the present study was to evaluate the utilization and diagnostic yields of CT pulmonary angiography (CTPA) using the Revised Geneva score and Wells’ criteria, in patients with suspected pulmonary embolism (PE). Methods: One hundred and twelve adult patients underwent CTPA for suspected PE were participated in this study. The outcome was positive or neg...
full textInvestigating the Relationship Between End Tidal Carbon Dioxide and Arterial Carbon Dioxide Pressure in Patients With Respiratory Distress Referred to the Emergency Room of Hazrat Rasool Akram Hospital
Background: Measuring End-Tidal Carbon Dioxide (ETCO-2) can be a non-invasive, fast, and reliable method to predict partial pressure of carbon dioxide (PaCO2) in patients with respiratory distress. This method, which can be a suitable substitute to measure PaCO2, is being used in many emergency rooms and operating rooms in developed countries, but its exact relationship with PaCO2 has not been ...
full textNoninvasive measurement of mean alveolar carbon dioxide tension and Bohr's dead space during tidal breathing.
The lack of methodology for measuring the alveolar carbon dioxide tension (PA,CO2) has forced investigators to make several assumptions, such as that PA,CO2 is equal to end-tidal (PET,CO2) and arterial CO2 tension (Pa,CO2). The present study measured the mean PA,CO2 and Bohr's dead space ratio (Bohr's dead space/tidal volume (VD,Bohr/VT)) during tidal breathing. The method used is a new, simple...
full textcorrelation of end-tidal carbon dioxide with arterial carbon dioxide in mechanically ventilated patients
background: patients undergone mechanical ventilation need rapid and reliable evaluation of their respiratory status. monitoring of end-tidal carbon dioxide (etco2) as a surrogate, noninvasive measurement of arterial carbon dioxide (paco2) is one of the methods used for this purpose in intubated patients. objectives: the aim of the present trial was to study the relationship between end-tidal c...
full textSteady-state end-tidal alveolar dead space fraction and D-dimer: bedside tests to exclude pulmonary embolism.
STUDY OBJECTIVE Less than 35% of patients suspected of having pulmonary embolism (PE) actually have PE. Safe bedside methods to exclude PE could save health-care resources and improve access to diagnostic testing for suspected PE. In patients with suspected PE, we sought to determine the sensitivity, specificity, and negative predictive value of (1) a steady-state end-tidal alveolar dead space ...
