Single-stage, off-pump hybrid repair of extensive aneurysms of the aortic arch and the descending thoracic aorta.

چکیده
S urgical treatment of aortic arch aneurysms has been challenging. These aneurysms are usually treated with open surgical repair that requires cardiopulmonary bypass (CPB) and hypothermic circulatory arrest (HCA). Furthermore, in cases of aneurysms involving both the aortic arch and the descending thoracic aorta (DTA) extensive surgical incisions or staged procedures are often necessary, increasing the perioperative risk. The purpose of this report is to describe the use of an innovative, hybrid technique that avoids the use of CPB and HCA in a single-stage surgical treatment for highrisk patients with extensive aortic arch and DTA aneurysms and a non-aneurysmal ascending aorta (Figure 1). This technique employs surgical debranching and revascularization of the aortic arch, and placement of endovascular stent-grafts within the ascending aorta, the aortic arch and the DTA.

A Single-Stage Repair of Arch and Descending Thoracic Aortic Aneurysms Using Jotec E-vita Open Plus Hybrid Stent Graft Combined With Antegrade Deployment of Thoracic Endograft.
We report a unique case of a 63-year-old female with extensive peripheral vascular disease who underwent a single-stage surgical repair of the aortic arch and descending thoracic aortic aneurysm utilizing the Jotec E-vita Open Plus hybrid stent graft system combined with antegrade deployment of a thoracic endograft via a median sternotomy.

The frozen elephant trunk (FET) technique is a modification of the traditional elephant trunk method, which was introduced by Borst in 1983 in order to treat extensive thoracic aortic aneurysms. The crux of the new method is the different type of aortic prosthesis, consisting of a Dacron part (with or without branches leading to the arterial vessels which exit the aortic arch) and a port for ex...

Abstract Traumatic descending thoracic aorta pseudo aneurysms have been treated traditionally with open surgery in the past, which have had noticeable rates of mortality and morbidity. A safer method of treatment for this disease is made possible with recent progress in endovascular treatment techniques. In this article, we present three cases of Traumatic descending thoracic aorta pseudo aneu...
متن کاملCombined valve-sparing root replacement and total arch replacement with frozen elephant trunk.
We report a case of simultaneous repair of an extensive thoracic aortic aneurysm from the aortic root to the distal aortic arch. A 54-year-old male had annuloaortic ectasia and a transverse aortic and distal arch aneurysm. Aneurysms of the descending aorta and the abdominal aorta were also demonstrated. The patient underwent aortic valve-sparing root reconstruction, replacement of the aortic ar...
متن کاملIs total debranching a safe procedure for extensive aortic-arch disease? A single experience of 27 cases.
OBJECTIVE Thoracic, arch, and proximal descending thoracic aorta diseases are still considered an enormous challenge. The hybrid approach developed in recent years (supra-aortic trunks debranching and thoracic endovascular repair aortic repair; TEVAR) may improve the morbidity and mortality of the population at risk. The aim of this study was to analyze retrospectively our experience in the hyb...
