The effects of induced hypertension on cerebral perfusion during delayed cerebral ischaemia in aneurysmal subarachnoid haemorrhage: a randomised clinical trial

چکیده
Methods Patients with aSAH and occluded aneurysm who developed clinical signs of DCI were randomised to either induced hypertension or no induced hypertension. Hypertension was induced with norepinephrine. Cerebral perfusion was assessed with CTP as CBF (ml/100g/s) in predefined regions (ROIs) on two time points: CTP1) at time of clinical deterioration and CTP2) 24 36 hours after randomisation. CBF was assessed by an observer blinded for treatment allocation. We assessed the difference in overall and lowest CBF per treatment group (Wilcoxon Signed Rank test) and the change in overall CBF (delta overall CBF) between the treatment groups (Mann Whitney U test).

Effects of Induced Hypertension on Cerebral Perfusion in Delayed Cerebral Ischemia After Aneurysmal Subarachnoid Hemorrhage: A Randomized Clinical Trial.
BACKGROUND AND PURPOSE The presumed effectiveness of induced hypertension for treating delayed cerebral ischemia after aneurysmal subarachnoid hemorrhage is based on uncontrolled case-series only. We assessed the effect of induced hypertension on cerebral blood flow (CBF) in aneurysmal subarachnoid hemorrhage patients with delayed cerebral ischemia in a randomized clinical trial. METHODS Aneu...
متن کاملAdvances in the understanding of delayed cerebral ischaemia after aneurysmal subarachnoid haemorrhage
Delayed cerebral ischaemia has been described as the single most important cause of morbidity and mortality in patients who survive the initial aneurysmal subarachnoid haemorrhage. Our understanding of the pathophysiology of delayed cerebral ischaemia is meagre at best and the calcium channel blocker nimodipine remains the only intervention to consistently improve functional outcome after aneur...
متن کاملEffects of Induced Hypertension on Cerebral Perfusion in Delayed Cerebral Ischemia After Aneurysmal Subarachnoid Hemorrhage
Delayed cerebral ischemia (DCI) contributes to poor outcome after aneurysmal subarachnoid hemorrhage and may occur when the cerebral blood flow (CBF) no longer meets the demand of the brain tissue in the setting of impaired cerebrovascular autoregulation. Vasospasm is an important cause of DCI, and for many decades, the so-called triple-H therapy is used to target this potential cause of DCI. O...
متن کاملMagnesium for aneurysmal subarachnoid haemorrhage (MASH-2): a randomised placebo-controlled trial
BACKGROUND Magnesium sulphate is a neuroprotective agent that might improve outcome after aneurysmal subarachnoid haemorrhage by reducing the occurrence or improving the outcome of delayed cerebral ischaemia. We did a trial to test whether magnesium therapy improves outcome after aneurysmal subarachnoid haemorrhage. METHODS We did this phase 3 randomised, placebo-controlled trial in eight cen...

The objective of this review is to review the anaesthetic implications of vasoactive compounds particularly with regard to the cerebral circulation and their clinical importance for the practicing anaesthetist. Material was selected on the basis of validity and application to clinical practice and animal studies were selected only if human studies were lacking. Hypotensive drugs have been used ...
