Ultrasound guided paravertebral catheter placement and potential future improvements

چکیده
A patient suffering with traumatic injuries including multiple rib fractures was excessively narcotized potentially impeding her recovery. A continuous infusion of local anaesthetic into the paravertebral space provided some opioid sparing effect, although the analgesic effect was less clear. The catheter was placed into the paravertebral space using a through-needle technique with ultrasound guidance. A discussion of the anatomical issues, including the endothoracic fascia is undertaken. Features of ultrasound imaging, needle guidance and catheter location are discussed Potential improvements to the technique of ultrasound-guided catheter placement are suggested, including speculative technological innovations that have yet to be fully evaluated but which may be of relevance. These include alternative ultrasound transducers, in-plane and out-of-plane needle approaches, modern highly echogenic needles, stimulating and coiling catheters, pressure transduction, 4D ultrasound and needle tracking sensors.

Ultrasound-guided paravertebral puncture and placement of catheters in human cadavers: an imaging study.
BACKGROUND During paravertebral block, the anterolateral limit of the paravertebral space, which consists of the pleura, should preferably not be perforated. Also it is possible that, during the block, the constant superior costotransverse ligament can be missed in the loss-of-resistance technique. We therefore aimed to develop a new technique for an ultrasound-guided puncture of the paraverteb...
متن کاملUltrasound-guided thoracic paravertebral puncture and placement of catheters in human cadavers: where do catheters go?
BACKGROUND Paravertebral regional anaesthesia is used to treat pain after several surgical procedures. This study aimed to improve on our first published ultrasound-guided approach to the paravertebral space (PVS) and to investigate a possible discrepancy between the needle, catheter, and contrast dye position. METHODS In 10 cadavers, we conducted 26 ultrasound-guided paravertebral approaches...
متن کاملUltrasound Guidance Is Helpful for Paravertebral Block Performance and Catheter Placement in Patients with Laminectomy after Thoracotomy or Lumbotomy: A Case Series Imaging Study
Today, the ultrasound guidance (USG) in regional anesthesia is gold standard more and more often used for medial or paramedian approaches around to the spine, such as the paravertebral (PV) block. Local anatomical changes may greatly handicap the performance of this type of block. We present clinical, sonographic, and radiological data on successful PV block and catheter placement in four patie...
متن کاملParavertebral blocks.
Paravertebral blocks have been demonstrated to represent an interesting alternative to epidural, especially for the management of perioperative and trauma pain. Initially performed mostly as single-shot blocks for breast surgery, thoracotomy, and hernia repairs in adults and children, presently these blocks are also used for placement of a paravertebral catheter, either unilateral or bilateral....
متن کامل9 Ultrasound Guided Vascular Access
Vascular access is an important procedure for clinicians to master. Chronic medical conditions, intravenous drug use, and obesity can all make placements of vascular catheters in central and peripheral veins challenging and time-consuming. External landmarks have traditionally guided the placement of the needle for the performance of central venous catheterization. The proximity of such structu...
