چکیده

We present the case of a 21 year-old woman with unremarkable medical history except for tachycardia episodes. The ECG showed a delta wave pre-excitation by a left lateral pathway. One radiofrequency pulse temporarily terminated the accessory pathway conduction, revealing a nodal conduction with complete left bundle branch block.

Successful Catheter Ablation as a Substitute for Cardiac Resynchronization Therapy in Patient with an Accessory Pathway-induced Cardiomyopathy

A 50-year-old man presented with exertional dyspnea and orthopnea. An electrocardiogram showed a delta wave and a wide QRS complex, similar to left bundle branch block. Cardiac echocardiography revealed diffuse severe hypokinesis and dyssynchrony. The patient was diagnosed with congestive heart failure. We considered that the patient's condition was caused by an accessory pathway-induced cardio...

Successful catheter ablation to accessory atrioventricular pathway as cardiac resynchronization therapy in a patient with dilated cardiomyopathy.

A 55-year-old man was admitted to our hospital for further examination of the abnormalities of chest X-ray and electrocardiogram. He was diagnosed with type B Wolff-Parkinson-White syndrome concomitant with dilated cardiomyopathy. Despite the medical therapy using enalapril and carvedilol for 20 months, his cardiac performance and brain natriuretic peptide (BNP) were not so improved. Because as...

Wide complex tachycardia in a healthy young man.

We describe a case of a young man referred with a broad complex tachycardia for evaluation and ablation. Electrophysiological study was performed and revealed a left lateral accessory pathway with antegrade decremental conduction causing antidromic reciprocating re-entrant tachycardia. Ablation was successfully performed. Left lateral accessory pathways with decremental conduction are rarely en...

A simple method to ablate left-sided accessory pathways in a patient with coronary sinus ostial atresia and persistent left superior vena cava: A case report

Introduction Wolff-Parkinson-White (WPW) syndrome is characterized by a short PR interval, wide QRS complex, and paroxysmal supraventricular tachycardia. An accessory atrioventricular connection is known to be the underlying mechanism of this syndrome. Accessory pathways (APs) are mainly located along the tricuspid or mitral annulus, accounting for 10%– 20% and 50%–60% of cases, respectively. M...

Decreased amplitude of left ventricular posterior wall motion with notch movement to determine the left posterior septal accessory pathway in WolV-Parkinson-White syndrome

Objective—To determine preoperatively, by analysing asynchronous left ventricular wall motion, whether to approach through the right ventricle or the left ventricle when carrying out catheter ablation of the accessory pathway in WolVParkinson-White syndrome, especially in patients with the pathway located on the septum. Methods—73 patients with manifest WolV-Parkinson-White syndrome who underwe...
