چکیده

medialis obliquus, is the main passive stabilizer of the patellofemoral joint. Recently, it has been shown that the medial patellofemoral complex is the main pathoanatomy after patellar dislocation [1-3] . In addition, biomechanical studies have demonstrated that the MPFL is the main restraint against lateral patellar displacement and that repair or reconstruction of the MPFL restores normal patellar tracking [4-8] . Since it is known that distal realignment procedures do not restore patellofemoral stability and can even increase the medial patellofemoral pressure, leading to arthrotic degeneration [9] , surgical interventions have focused on the passive joints stabilizers. Furthermore, lateral release procedures have been shown to increase patellofemoral instability in biomechanical investigation and thus reconstruction or repair of the MPFL has became the preferred treatment [10] .

Guidelines for medial patellofemoral ligament reconstruction in chronic lateral patellar instability.

The standard surgical approach for chronic lateral patellar instability with at least two documented patellar dislocations is to stabilize the patella by using an anatomic medial patellofemoral ligament reconstruction with a mini-open technique and a graft that is stronger than the native ligament to compensate for the uncorrected predisposing factors underlying patellar instability. Even thoug...

Patellofemoral Joint Instability: A Review of Current Concepts

The patellofemoral joint remains the enigma of orthopedics and sports medicine. Patellar dislocation is a common problem in the younger and athletic population and it is more disabling than cruciate ligament injuries. The pathology is often multifactorial and complex with no one factor being the sole defining etiology. The current management of patella dislocation has been linked with poor pati...

Reconstruction of the medial patellofemoral ligament for patellar instability using an autologous gracilis tendon graft.

Patellofemoral dislocations are common. In cases with recurrence or residual instability, surgical intervention is usually considered. Numerous treatment protocols have been used in the past to treat patellofemoral instability secondary to patella dislocation. Reconstruction of the medial patellofemoral ligament is one of the possible options, since it was acknowledged to have a major medial st...

Outcomes of combined tibial tuberosity transfer and medial patellofemoral ligament reconstruction for recurrent patellar instability.

Patellofemoral instability is multifactorial and is associated with pathomechanics secondary to anatomical variance. Surgical management of this problem must be tailored to each patient and a thorough clinical and radiological assessment of the anatomical alignment should be carried out pre-operatively. The aim of this study is to assess the role of medial patellofemoral ligament reconstruction...

Biomechanical Comparison of Medial Patellofemoral Ligament Reconstuction and Distal Realignment for Patellar Instability

In recent years, the medial patellofemoral ligament (MPFL) has been considered as the primary restraint of the medial patellar stabilizers [1]. Clinical reports have revealed that MPFL injury occurs in almost all patients with traumatic patellar dislocation, and attention has been focused on the repair and reconstruction of the MPFL in recurrent patellar dislocation and patellar subluxation [2]...
