چکیده

■ EVIDENCE-BASED ANSWER There is no compelling evidence that outpatient diagnostic workup of patients with transient ischemic attack (TIA) is less safe than inpatient workup, or that hospitalization prevents stroke or improves stroke outcomes after TIA (strength of recommendation [SOR]: C, based on case series studies). Because the risk of stroke is substantial in the week following a TIA (SOR: A, based on a prospective cohort study), evaluation and treatment for reversible stroke risk factors should be initiated urgently and completed within a week of initial presentation (SOR: C, based on expert consensus opinion). Risk factors for patients at highest risk for stroke or other cardiovascular events after TIA include age >60 years, diabetes, TIA lasting longer than 10 minutes, and a TIA associated with weakness or speech impairment (SOR: B, based on retrospective cohort study). Hospitalization may be prudent for patients at high risk for cardiovascular events or for those with mental status changes, an inadequate home situation, or the physician’s inability to obtain expedient evaluation (SOR: C, based on case series studies).

Prevention of recurrent ischemic stroke.

Recurrent ischemic stroke and transient ischemic attack are common problems in primary care, with stroke survivors averaging 10 outpatient visits per year. Risk factors such as hypertension, diabetes, and hypercholesterolemia should be evaluated during each office visit. Attention should be given to lifestyle modification including management of obesity, smoking cessation, reduction in alcohol ...

ABCD2 scores and prediction of noncerebrovascular diagnoses in an outpatient population: a case-control study.

BACKGROUND AND PURPOSE Among patients with transient ischemic attack, the ABCD2 score predicts short-term stroke risk. Use of the ABCD2 score assumes the underlying diagnosis to be transient ischemic attack; however, most transient ischemic attack services assess a variety of diagnoses. We hypothesized that patients with low ABCD2 score predominantly have noncerebrovascular diagnoses. METHODS...

Organized outpatient care: stroke prevention clinic referrals are associated with reduced mortality after transient ischemic attack and ischemic stroke.

BACKGROUND AND PURPOSE Organized inpatient stroke care decreases mortality and morbidity irrespective of patient age, stroke severity, or stroke subtype. Limited information is available on whether organized outpatient care models such as stroke prevention clinics (SPC) improve outcomes after a transient ischemic attack or ischemic stroke. We compared 1-year mortality and stroke readmission in ...

Should magnetic resonance imaging or computed tomography be the primary brain imaging modality in the transient ischemic attack clinic?

OBJECTIVE The Royal College of Physicians and National Institute of Clinical Excellence have recommended that magnetic resonance imaging should be the modality of choice for cerebral imaging in transient ischemic attack patients. However, implementation of this is often difficult. Therefore, it is important to know whether magnetic resonance imaging contributes significantly to early clinical m...

Different aspects of transient ischemic dilation

Transient ischemic left ventricular dilation (TID) is a marker of severe and extensive coronary artery disease as well as an increased risk of adverse outcomes. The patients with more severe and extensive ischemia, multivessel-type of perfusion abnormality as well as patients with left anterior descending artery (LAD) territory perfusion abnormality have more probab...
