چکیده

In October 2012, the CDC reported an outbreak of fungal meningitis associated with methylprednisolone injections [1]. Although the index case had Aspergillus fumigatus meningitis [2], subsequent reports indicated that the majority of affected patients had meningitis caused by Exserohilum rostratum, an extremely rare cause of human fungal disease [1,3,4]. The outbreak is presumably the result of direct inoculation of E. rostratumcontaminated methylprednisolone into human tissue, and the cause appears to be a manufacturing mishap. This is not the first instance of fungal meningitis resulting from contaminated steroid preparations. It was 10 years ago when several cases of fungal meningitis caused by Exophiala (Wangiella) dermatitidis occurred in patients receiving injectable steroids [5]. Further back in time, epidemics of zygomycosis occurred from the use of bandages and wood depressors, among others, that were contaminated with fungal spores [6]. Each of these outbreaks was characterized by high mortality and morbidity, thereby illustrating the potential of certain fungal species to cause life-threatening disease when they interact with human hosts in whom integument and mucosal defenses have been compromised by iatrogenic procedures.

Fungal Contamination of Methylprednisolone Causing Recurrent Lumbosacral Intradural Abscess

Fungal meningitis transmitted through injections of methylprednisolone contaminated with Exserohilum rostratum affected 753 persons and caused 61 deaths in the United States in 2012. We report a case of infection recurrence after 24-months with the unique manifestation of an intradural fungal abscess. Fungal disease should remain on the differential diagnosis list for previously exposed patients.

Spinal and Paraspinal Infections Associated with Contaminated Methylprednisolone Acetate Injections — Michigan, 2012–2013

As of May 6, 2013, Michigan had reported 167 (52%) of the 320 paraspinal or spinal infections without meningitis associated with the 2012-2013 fungal meningitis outbreak nationally. Although the index patient had a laboratory-confirmed Aspergillus fumigatus infection, the fungus most often identified, including in unopened vials of methylprednisolone acetate (MPA), remains Exserohilum rostratum...

Utility of real-time PCR for detection of Exserohilum rostratum in body and tissue fluids during the multistate outbreak of fungal meningitis and other infections.

Exserohilum rostratum was the major cause of the multistate outbreak of fungal meningitis linked to contaminated injections of methylprednisolone acetate produced by the New England Compounding Center. Previously, we developed a fungal DNA extraction procedure and broad-range and E. rostratum-specific PCR assays and confirmed the presence of fungal DNA in 28% of the case patients. Here, we repo...

Detection of fungal DNA in human body fluids and tissues during a multistate outbreak of fungal meningitis and other infections.

Exserohilum rostratum was the major cause of an outbreak of fungal infections linked to injections of contaminated methylprednisolone acetate. Because almost 14,000 persons were exposed to product that was possibly contaminated with multiple fungal pathogens, there was unprecedented need for a rapid throughput diagnostic test that could detect both E. rostratum and other unusual agents of funga...

Fungal meningitis due to contaminated epidural steroid injections.

In the fall of 2012, as we finalized this issue of , an unprecedented outbreak of fungal meningitis occurred that was caused by contaminated preservative-free methylprednisolone acetate solution from the New England Compounding Center used in epidural steroid injections in thousands of patients. The predominant pathogen was found to be Exserohilum rostratum (a black mold). Aspergillus fumigatus...
