Orbital pseudotumor

چکیده
A 42-year-old male presented to the emergency department with the complaint of irritation in the left eye for 1.5 months. He also complained of decreased and blurry vision in the left eye. He had a history of non-insulindependent diabetes mellitus (NIDDM), hypertension, and bipolar disorder and was noncompliant with his medications. His visual acuity was decreased on the left. His pupils were 3 mm, symmetric, and equally reactive. His extraocular muscles were intact. The left eye was grossly proptotic and his conjunctiva are injected bilaterally. On fundoscopic exam, the vasculature was prominent. The cup to disk ratio was grossly normal. There was a small amount of purulent discharge noted from his medial canthus. The remainder of his examination is unremarkable. Orbital pseudotumor (an idiopathic orbital inflammation), a diagnosis of exclusion, is the third most common orbital disease, accounting for 5–6% of orbital disorders [1]. It is most common in the 4th–5th decade of life, unilateral or bilateral, and has no gender predilection. The most common presentation is proptosis. Other common symptoms include ocular motor deficits, pain, lid swelling or lid mass, ptosis, and chemosis [2]. Computed tomography (CT), the preferred method of imaging [3], and ultrasound of the orbits will show inflammation of the eye wall. Magnetic resonance imaging (MRI) is recommended for evaluation of the cavernous sinuses [4]. At times, biopsy is required to determine the etiology of the disease. The differential is extensive and orbital cellulitis must be considered. This condition typically responds to steroids however recurrence is likely. Urgent referral to ophthalmology is recommended. Introduction: Orbital pseudotumor, also known as idiopathic orbital inflammatory syndrome (IOIS), is a benign, non- infective inflammatory condition of the orbit without identifiable local or systemic causes. The disease may mimics a variety of pathologic conditions. We present a case of pseudotumor observed in a patient admitted under the name of orbital cellulities. Case Report: A 26-year...
متن کاملInflammatory Pseudotumor Of The Orbit: A Histopathologic And Immunohistochemical Study Of 32 Cases
Background and Objective: Inflammatory pseudotumor is a lesion composed of proliferating spindle cells with mixed inflammatory infiltrates. Some authors have proposed the name inflammatory myofibroblastic tumor as a proper descriptive term rather than the vague inflammatory pseudotumor. The aim of this study was to verify the myofibroblastic origin of spindle cells in idiopathic orbital inf...

Ophthalmologists in the early 1800s made an interesting observation that several patients with presumed orbital tumors showed spontaneous improvement without any treatment. Until then proptosis was considered as prima facie of orbital neoplasm. Panas coined the term “pseudoplasm” for these puzzling cases. Birchfield in 1930 used the term orbital pseudotumor for such cases. Improvements in diagn...
متن کاملOrbital pseudotumor as a result of chronic sinusitis in an HIV‐positive patient
Orbital inflammatory pseudotumor is an infrequent lesion that may mimic malignancy and may be associated with autoimmune disease. Orbital pseudotumor is in the differential in any patient with an orbital mass, especially in the setting of chronic infection or immunosuppression. This HIV-positive patient presented with chronic sinusitis and orbital pseudotumor.
متن کاملAtypical Presentation of Orbital Pseudotumor with Visual Loss as an Initial Manifestation
BACKGROUND An orbital pseudotumor typically presents with periorbital pain, cranial nerve palsies and proptosis. Although visual deterioration is not unexpected in this pathology, its presentation solely with visual loss is unusual. CASE REPORT In this short report, we summarize a case of orbital pseudotumor which presented solely with a decrease in visual acuity, and discuss the clinical and...
متن کاملInflammatory Pseudotumor of the Head Presenting with Hemiparesis and Aphasia
Inflammatory pseudotumor most commonly occurs in the orbit and produces orbital pseudotumor, but extension into brain parenchyma is uncommon. We report a case of inflammatory pseudotumor involving sphenoid sinus, cavernous sinus, superior orbital fissure, orbital muscle, and intracranial extension into left temporal lobe producing right hemiparesis and wernicke's aphasia. The patient improved c...
