Aldosterone antagonism and myocardial infarction: from animals to man and back.

چکیده
Inhibiting the renin-angiotensin system has proven to be one of the most fruitful therapeutic strategies in cardiovascular medicine. Believed to have evolved to maintain blood volume and perfusion pressure in conditions of hemorrhage or hypovolemia, the renin-angiotensin-aldosterone system (RAAS) is upregulated in the setting of left ventricular (LV) dysfunction or heart failure (1). Pharmacologic inhibitors of this system—including renin inhibitors, angiotensinconverting enzyme (ACE) inhibitors, aldosterone antagonists, and angiotensin receptor blockers—were all initially designed for the treatment of hypertension, and, with the exception of renin inhibitors, are currently clinically used for this indication. Nevertheless, the benefits of inhibiting the RAAS have extended well beyond this initial therapeutic target.

Relationship between fingerprint dermatoglyphics in association with suceptibility to myocardial infarction in man
coronary heart diseases is one of the major causes of mortality, and family history is one of the major risk factors.fingerprint tupes are unique genetically based characteristic for each indivdual.in this study fingerprints of 100 ml patients (test) in CCU departments were compared to 100 patients from other hospital wards (control) who had no history of MI. the mean weight of test group was 64...
متن کاملEffect of a selective aldosterone receptor antagonist in myocardial infarction.
Myocardial infarction (MI) initiates adaptive tissue remodeling, which is essential for heart function (such as infarct healing) but is also important for maladaptive remodeling (for example, reactive fibrosis and left ventricular dilation). The effect of aldosterone receptor antagonism on these processes was evaluated in Sprague-Dawley rats using eplerenone, a selective aldosterone receptor an...
متن کاملSilent Myocardial Infarction Presented with Homonymous Hemianopia: A Rare Case Study
Silent myocardial infarction is a little-known phenomenon, the mechanisms of which have still remained unclear. Herein, we presented the case of a middle-aged man suffering from silent myocardial infarction who presented with homonymous hemianopia and no other major cardiovascular risk factors, except for stage 1 hypertension.

PURPOSE OF REVIEW This review highlights recent clinical studies demonstrating the contribution of aldosterone to cardiovascular mortality, vascular dysfunction, and renal injury in the context of advances in our understanding of the molecular biology of aldosterone. RECENT FINDINGS Mineralocorticoid receptor antagonism reduces mortality in patients with congestive heart failure and following...
متن کاملMyocardial damage of the entire ventricular region in a patient with acute myocardial infarction
Technetium-99m-pyrophosphate (99mTc-PYP) has been used, in combination with thallium-201, to estimate the site and extent of myocardial infarcts. We report a case of acute myocardial infarction with severe coronary disease in which the distribution of 99mTc-PYP was extensive. A 78-year-old man presented with dyspnea, and a diagnosis of non-ST-segment elevation acute myocar...
