Management of myasthenic crisis.

چکیده
Sir, I read with interest the recent article by Murthy et al. It provides excellent data regarding the management and outcome of patients with myasthenic crisis. I would like to make certain observations. First, Murthy et al have utilized the neostigmine test for confirmation of diagnosis. I would like to emphasize that the edrophonium (Tensilon) test is an excellent alternative for this purpose. It may even be better as the test result is obvious in 1-2 minutes, as compared to neostigmine that requires 15-30 minutes. Tensilon test has a relatively high sensitivity and specificity, and the incidence of serious complications is only 0.16%. Secondly, it is important to differentiate myasthenic crisis from cholinergic crisis. The edrophonium test allows for this differentiation and this may help in optimizing the treatment. Thirdly, Murthy et al started all their patients on steroids prior to discharge. We agree with the approach, however, caution is required as steroids may exacerbate weakness in a subgroup of patients with myasthenia. Therefore, it is advisable to keep patients hospitalized after initiating steroids till clinical improvement begins, as exacerbations after that are unusual. Finally, Murthy et al have shown the effectiveness of lowvolume plasma exchanges in myasthenic crisis, which is similar to our observations. It is particularly important in the Indian scenario to minimize the cost of treatment. It is encouraging to note that plasmapheresis was shown to be superior to intravenous immunoglobulins in treating myasthenic crisis in an earlier study.

Respiratory Care for Myasthenic Crisis
Myasthenic crisis is a life-threatening complication of myasthenia gravis (MG) that is traditionally defined as weakness of respiratory muscles that is severe enough to require intubation or artificial respiratory support (Jaretzki et al., 2000). Rapid and marked increase in limb and bulbar weakness without respiratory failure, however, should probably be also defined as crisis in a myasthenic ...

Myasthenia gravis is an autoimmune disorder of peripheral nervous system, leading to fluctuating muscle weakness. It is caused by circulating antibodies that block acetylcholine nicotinic postsynaptic receptors at the postsynaptic neuromuscular junction. Myasthenic crisis is a life-threatening complication, which is defined as weakness from acquired myasthenia gravis. In this paper we described...

Myasthenia gravis (MG) is an autoimmune disorder affecting neuromuscular transmission leading to generalized or localized muscle weakness due most frequently to the presence of autoantibodies against acetylcholine receptors in the postsynaptic motor end-plate. Myasthenic crisis (MC) is a complication of MG characterized by worsening muscle weakness, resulting in respiratory failure that require...

متن کاملTracheoesophageal Fistula Caused by Tracheostomy in a Patient with Myasthenia Gravis after a Myasthenic Crisis
A 57-year-old woman with myasthenia gravis (MG), who had experienced a myasthenic crisis, complained of coughing while drinking. At first, this appeared to be a sequela of the myasthenic crisis. However, after further investigation, the problem was identified as a tracheoesophageal fistula, a complication of tracheostomy. Here, we describe this special case in the hope that we can improve diagn...
