Successful ablation of a left-sided pathway in patient with a left atrial coronary sinus orifice: a case report

چکیده
Coronary sinus (CS) is a landmark for cardiac intervention. Before catheter ablation of paroxysmal supraventricular tachycardia, coronary sinus is used to obtain recordings from the left atrium and ventricle. Despite asymptomatic, anomalies of coronary sinus may become an obstacle for electrophysiological catheterization. Here, we described a case of successful ablation of paroxysmal supraventricular tachycardia in which the accessory pathway located at the mitral valve annulus and the coronary sinus opened directly at left atrium. To the best of our knowledge, this is the first case reported that has left atrium catheterized, recorded and ablated through interatrial septum because of the left atrial coronary sinus orifice.

Atypical atrioventricular node reciprocating tachycardia masquerading as tachycardia using a left-sided accessory pathway.
OBJECTIVES The study was performed to document that atrioventricular node reciprocating tachycardia (AVNRT) can be associated with eccentric retrograde left-sided activation, masquerading as tachycardia using a left accessory pathway. BACKGROUND The eccentric retrograde left-sided activation during tachycardia is thought to be diagnostic of the presence of a left free wall accessory pathway. ...
متن کاملSuccessful Ablation of a Left-Sided Accessory Pathway in a Patient with Coronary Sinus Atresia and Arteriovenous Fistula: Clinical and Developmental Insights
BACKGROUND AND OBJECTIVES While radiofrequency ablation catheter ablation of accessory pathways is generally safe and effective, anatomic variants can cause considerable challenges in effecting cure. Our objective was to use an unusual case where coronary sinus was absent and arterial venous fistula was present and a left-sided pathway required mapping and ablation to develop a framework to app...
متن کاملInteratrial electrical dissociation after catheter-based ablation for atrial fibrillation and flutter.
A 58-year-old man with hypertension and diabetes with recurrent persistent symptomatic atrial fibrillation resistant to sotalol-facilitated cardioversions was referred for ablation. Catheter-based radiofrequency ablation for persistent atrial fibrillation was performed, which consisted of isolation of all 4 pulmonic veins; linear ablations along the left atrial roof; an ablation line from the l...
متن کاملNonreentrant atrioventricular nodal tachycardia due to triple nodal pathways manifested by radiofrequency ablation at coronary sinus ostium.
We report a case of complex supraventricular tachycardia manifested by radiofrequency delivery. Initially, the patient presented with orthodromic atrioventricular reentrant tachycardia via a left-sided accessory pathway that was successfully eliminated. Atrial tachycardia originating from coronary sinus ostium was also induced. Radiofrequency delivery at the coronary sinus ostium induced a narr...
متن کاملAtrial Arrhythmia Ablation in Adult Congenital Heart Disease with a Persistent Left-side Superior Vena Cava
We herein report a case of atrial tachycardia (AT) originating from the dilated coronary sinus (CS) connected to a persistent left superior vena cava. The earliest activation site of AT was localized at the superior CS ostium, identified using a novel ultra-rapid high-density mapping system (Rhythmia™). Successful ablation was performed at the corresponding position. This report demonstrated th...
