چکیده

Diagnosis of indeterminate mediastinal masses and staging of lung cancer poses a significant challenge. Transesophageal endoscopic ultrasound scanning (EUS) provides high resolution imaging of the mediastinum and offers the facility of fine needle aspiration or tru-cut biopsy under real-time ultrasound guidance. Accurate diagnosis of nodal involvement in lung cancer is pivotal to avoid unnecessary surgical exploration. The reported specificity of CT, MRI or PET is insufficient to exclude patients from curative management and mediastinoscopy or thoracoscopic sampling though specific requires general anaesthesia and hospital admission in addition to carrying 2-3% morbidity. EUS has a sensitivity of 60-90% and specificity of 70-100% in staging of lung cancer. For the staging of lung cancer, EUS has been found to be superior and specific as compared to CT, PET, mediastinoscopy as well as transbronchial aspiration. In other mediastinal lesions like tuberculosis or sarcoidosis, EUS is far more accurate in characterisation and obtaining tissue diagnosis than other contemporary modalities. EUS is thus emerging as the modality of choice is staging of lung cancer and diagnosis of mediastinal lesions.

Role of endoscopic ultrasound-guided fine-needle aspiration in lung and mediastinal lesions.

BACKGROUND Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) was initially introduced for diagnosing gastrointestinal and pancreatic lesions, and later on for lung and mediastinal lesions. It can provide tissue diagnosis of lung cancer where bronchoscopy is non-diagnostic. It is a minimally invasive method for lymph node (N) and metastasis (M) staging of non-small cell lung cancer, ...

Transesophageal endoscopic ultrasound-guided fine-needle aspiration for the mediastinal staging of extrathoracic tumors: a new perspective.

BACKGROUND Several extrathoracic tumors metastasize to the mediastinum. Mediastinoscopy is the standard method to obtain tissue proof of mediastinal spread, but drawbacks are its invasiveness, requirement for general anesthesia and costs. Transesophageal endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is indicated in lung cancer staging guidelines as a minimally invasive alternati...

Minimally invasive endoscopic staging of suspected lung cancer.

CONTEXT In patients with suspected lung cancer, the presence of mediastinal lymph node metastasis is a critical determinant of therapy and prognosis. Invasive staging with pathologic confirmation is recommended. Many methods for staging exist; mediastinoscopy, an invasive procedure requiring general anesthesia, is currently regarded as the diagnostic standard. OBJECTIVE To compare the diagnos...

Transbronchial and transoesophageal (ultrasound-guided) needle aspirations for the analysis of mediastinal lesions.

A tissue diagnosis of mediastinal nodes is frequently needed for accurate lung cancer staging as well as the assessment of mediastinal masses. Transbronchial needle aspiration (TBNA) is a safe procedure that is performed during routine bronchoscopy. Provided mediastinal metastases are confirmed, TBNA has a high impact on patient management. Unfortunately, TBNA remains underused in current daily...

Radial Probe Endobronchial Ultrasound for Peripheral Pulmonary Lesions: Initial Experience in an Indian Tertiary Healthcare Centre

Introduction : Diagnosis of peripheral pulmonary nodules is confusing; therefore, an accurate and safe lung biopsy can prevent unnecessary invasive diagnostic procedures. This study soughtto study the diagnostic yield, sensitivity, specificity, and negative and positive predictive values (NPV and PPV) of radial probe endobronchial ultrasound (EBUS)-guided biopsy for peripheral pulmonary lesions...
