Successful Treatment of Superior Vena Cava Syndrome Induced by Transvenous Permanent Pacemaker Implantation with Balloon Angioplasty

چکیده
Superior vena cava (SVC) syndrome is a rare but potentially lethal complication of transvenous permanent pacemaker implantation. The pathogenesis is unclear, but thrombosis and fibrotic constriction of the vessels are believed to be responsible for this situation. Multiple, severed, and previously infected leads may increase the risk of SVC syndrome, but are not prerequisite for it. Anticoagulation, thrombolytics, percutaneous balloon angioplasty with or without stent, and surgery have been used to solve the problem. We report a case who developed SVC syndrome three years after the implantation of a DDD pacemaker for sick sinus syndrome. Anticoagulation alone failed to relieve his symptoms. After successful balloon angioplasty, the patient remained asymptomatic one year after the procedure.

A case of pacing lead induced clinical superior vena cava syndrome: a case report
INTRODUCTION Transvenous pacing is a relatively safe treatment with a low complication rate, but serious thromboembolic complications have been reported to occur in 0.6% to 3.5% of cases. Superior vena cava obstruction syndrome is generally an uncommon but serious complication occurring in <0.1% of patients. However, when it occurs it carries with it significant morbidity and mortality. CASE ...
متن کاملImplantation of Transvenous Permanent Pacemaker in a Patient with Persistent Left Superior Vena Cava and Absent Right Superior Vena Cava
Vascular access has remained a major challenge for implantation of permanent pacemaker leads. A persistent Left Superior Vena Cava (LSVC), especially with an absent Right Superior Vena Cava (RSVC), is a rare finding during pacemaker implantation and is accompanied by technical difficulties. Herein, we describe a case of sinoatrial node arrest, in which finding a suitable vein for passing the le...
متن کاملPacemaker induced superior vena cava syndrome: a case report
INTRODUCTION Pacemaker induced superior vena cava syndrome is an unusual complication of pacemaker implantation. Endothelial damage caused by repeated trauma from the lead is thought to be responsible for the stenosis. Malignancy has been historically the most common etiology. However, the increase in use of indwelling venous catheters and cardiac pacemaker has resulted in more patients with su...
متن کاملPercutaneous treatment for pacemaker-associated superior vena cava syndrome.
Superior vena cava (SVC) obstruction is an uncommon but serious complication associated with permanent pacemaker implantation. The mechanical stress associated with pacemaker wires may lead to vessel wall inflammation, fibrosis, and thrombus formation, and ultimately to venous stenosis and occlusion. The surgical treatment of pacemaker related SVC syndrome requires thoracotomy and carries signi...

A patient with the sick sinus syndrome accompanying mirror-image dextrocardia which was associated with double superior venae cavae and an absent inferior vena cava is presented. A permanent transvenous demand pacemaker was inserted because of repeated episodes of dizziness and a documented 31/2-second period of asystole. Precise knowledge of the venous system and the location of the apex of th...
