A case-matched study of anatomical segmentectomy versus lobectomy for stage I lung cancer in high-risk patients.

چکیده
OBJECTIVE Sublobar resections may offer a method of increasing resection rates in patients with lung cancer and poor lung function, but are thought to increase recurrence and therefore compromise survival for stage I non-small cell lung cancer (NSCLC). To test this hypothesis we have compared the long-term outcome from lobectomy and anatomical segmentectomy in high-risk cases as defined by predicted postoperative FEV1 (ppoFEV1) less than 40%. METHODS Over a 7-year period 55 patients (27% of all resections for stage I NSCLC) with ppoFEV1<40% underwent resection of stage I NSCLC. The 17 patients who underwent anatomical segmentectomy were individually matched to 17 patients operated by lobectomy on the bases of gender, age, use of VATS, tumour location and respiratory function. We compared their perioperative course, tumour recurrence and survival. RESULTS There were no significant differences in hospital mortality (one case in each group), complications or hospital stay. Overall 5-year survival was 69%. There were no differences in recurrence rates (18% in both groups) or survival (64% after lobectomy and 70% after segmentectomy). There was preservation of pulmonary function after segmentectomy (median gain of 12%) compared to lobectomy (median loss of 12%) (P=0.02). CONCLUSIONS Anatomical segmentectomy allowed for surgical resection in patients with stage I NSCLC and impaired respiratory reserve without compromising oncological results but with preservation in respiratory function.

Survival following segmentectomy or lobectomy in elderly patients with early-stage lung cancer
PURPOSE To determine the survival following segmentectomy versus lobectomy in elderly patients with early-stage non-small cell lung cancer (NSCLC). METHODS We identified 12324 elderly (≥ 70 years) patients with stage I ≤ 3 cm NSCLC in the Surveillance, Epidemiology and End Results (SEER) database. Propensity score methods were used to balance baseline characteristics of patients undergoing se...
متن کاملSublobectomy versus Lobectomy for stage IA (T1a) non-small-cell lung cancer: a meta-analysis study
BACKGROUND Although lobectomy is considered the standard surgical treatment for the majority of patients with non-small-cell lung cancer (NSCLC), the operation project for patients with stage IA NSCLC (T1a, tumor diameter≤2 cm) remains controversial. Sublobectomy is appropriate only in certain patients as many doctors consider it to be overtreatment. We evaluated the five-year overall survival ...
متن کاملSegmentectomy versus lobectomy for clinical stage IA lung adenocarcinoma.
BACKGROUND Despite the increasing prevalence of the early discovery of small-sized non-small cell lung cancers (NSCLCs), particularly adenocarcinoma, sublobar resection has not yet gained acceptance for patients who can tolerate lobectomy. METHODS We compared the outcomes of segmentectomy (n=155) and lobectomy (n=479) in 634 consecutive patients with clinical stage IA lung adenocarcinoma and ...
متن کاملSegmentectomy as a safe and equally effective surgical option under complete video-assisted thoracic surgery for patients of stage I non-small cell lung cancer
BACKGROUND While video-assisted thoracic surgery lobectomy has been widely accepted for the treatment of non-small cell lung cancer, the debate over video-assisted thoracic surgery segmentectomy still remains. This study compared the clinical outcomes using the two procedures for stage I non-small cell lung cancer patients. METHODS Retrospective review was conducted on patients who underwent ...
متن کاملVideo-assisted thoracoscopic lobectomy versus open lobectomy for clinical stage I non small cell lung cancer: a case-control study
This study aimed to compare outcomes following video-assisted thoracoscopic lobectomy (VATS) versus open lobectomy for clinical stage I non small cell lung cancer at a China center. Eighty-one consecutive patients who underwent video-assisted thoracoscopic lobectomy from January 2007 to December 2013 were compared with 94 patients undergoing open surgery during the same time period. Patients we...
