چکیده

Few multi-centre studies of the impact of computerised provider order entry (CPOE) systems on health care efficiency and effectiveness exist. Further, demonstrating a link between system use and improvements in patient outcomes is challenging. An often neglected step is to characterise the nature of the problem prior to CPOE introduction to ensure that the 'problem' being addressed has a demonstrated impact on the outcome of interest. We undertook a two-staged project to i) investigate the link between test turnaround time (TAT) and length of stay for emergency department patients prior to CPOE; ii) to measure the impact of CPOE on TAT in four Australian hospitals to examine the consistency of findings. We found TAT is a significant contributor to length of stay. All four hospitals experienced a significant reduction in TAT following CPOE. This study presents evidence that TAT is directly related to length of stay and that CPOE systems are effective at reducing TAT. These results add weight to the hypothesis that the introduction of CPOE may positively impact upon patient outcomes.

Computerised Order Entry Systems: Sustained Impact on Laboratory Efficiency and Mortality Rates?

Few studies have attempted to measure the effectiveness of computerised test-order entry systems to reduce test turnaround time and the extent to which improvements are sustained or continue over time. Further, a recent study has raised the possibility that such systems, which require significant work practice change, may be associated with an increase in mortality rates. Our study answered two...

A survey-based inventory of clinical decision support systems in computerised provider order entry in Swiss hospitals.

QUESTIONS UNDER STUDY Computerised order entry and clinical decision support systems (CDSSs) are considered to be important strategies to safeguard drug treatment. Although their dissemination is nationally enforced in Switzerland, current implementation rates vary. Moreover, types and implementation modes of CDSSs can differ substantially. This study maps the implementation of computerised ord...

The impact of computerized provider order entry systems on medical-imaging services: a systematic review

BACKGROUND Computerized provider order entry (CPOE) systems have been strongly promoted as a means to improve the quality and efficiency of healthcare. METHODS This systematic review aimed to assess the evidence of the impact of CPOE on medical-imaging services and patient outcomes. RESULTS Fourteen studies met the inclusion criteria, most of which (10/14) used a pre-/postintervention compa...

Efficiency Gains with Computerized Provider Order Entry

Objective: The objective of this project was to measure efficiency gains in turnaround times with the implementation of a computerized provider order entry (CPOE) system. Methods: Preand post-CPOE turnaround times (TATs) were measured for orders placed for laboratory, radiology, and pharmacy. The pre-CPOE group was nonrandomized and included a convenience sample of 240 patients with a sample of...

The Evaluation of Error Types and Turnaround Time of Preanalytical Phase in Biochemistry and Hematology Laboratories

Background& Objective: Each laboratory should determine the type of errors and turnaround time (TAT), especially in the preanalytical phase to report quality and timeliness of the test results. The current study aimed at investigating the common causes of preanalytical errors in biochemistry and hematology laboratories and evaluating the preanalytical ...
