چکیده

ABSTRACT Balloon atrial septostomy or Rashkind is a technique described almost half a century ago to dilate a preexisting atrial communication in order to enhance atrial mixing and to decompress the left atrium in congenital heart disease. With the contribution of fetal echocardiography this technique can be pre-planed, but, still many complications can arrive. It is done almost routinely in all case of transposition of the great arteries with restrictive interatrial communication. Many other techniques developed, but Rashkind intervention remains a reference in congenital heart interventions in newborn or small infant. It is a challenging procedure that needs trained interventional/congenital cardiologists and a well prepared catheterization laboratory, with the possibility for surgical or circulatory back-up. Nowadays, few complications can arrive, but are not to be neglected.

سپتوستومی دهلیزی با راهنمایی اکوکاردیوگرافی: گزارش هشت بیمار

Background: Balloon atrial septostomy is an emergent procedure in pediatric cardiology. Nowadays, most patients in need of the procedure have acceptable outcomes after surgical repair. Thus, it is important to perform this procedure as safe as possible. By performing early arterial switch operation and prostaglandin infusion, the rate of balloon atrial septostomy has markedly decreased. Howeve...

Clinical use of blade atrial septostomy.

A cardiac catheter enclosing an extensible blade was used to enlarge the interatrial opening in seven patients. Two patients with transposition of the great arteries who had balloon atrial septostomy as newborns subsequently presented with clinical evidence of a restrictive interatrial opening at 1 and 4 months of age. Cardiac catheterization confirmed restenosis of the interatrial opening and ...

Use of prostaglandin E2 in management of transposition of great arteries before balloon atrial septostomy.

Fifteen infants with transposition of the great arteries and severe hypoxaemia were treated with prostaglandin E2 infusions before atrial septostomy was performed. Twelve patients had simple transposition and three had small ventricular septal defects. The infusion resulted in a highly significant increase of PaO2 from 22 +/- 3 mmHg to 37 +/- 5 mmHg within one to two hours. Only one patient did...

Balloon atrial septostomy under two dimensional echocardiographic control.

Two dimensional echocardiography has been used to guide balloon atrial septostomy in five cases of transposition of the great arteries. A single lumen balloon catheter was used. The catheter can be accurately located within the left atrium before inflation of the balloon, thus avoiding some of the traumatic accidents possible during septostomy. The balloon can be observed creating the atrial de...
