چکیده

Background Knowledge of caval contribution to flow in the Right and Left Pulmonary Artery (RPA and LPA) of Fontan patients is of great interest, because uneven flow distribution to the lungs is associated with pulmonary arteriovenous malformations. However, in Fontan circulation more than 1 source of flow to the RPA and LPA is present, which makes difficult to quantify the flow distribution with standard methods. Although a novel MRI application was developed to quantify caval contribution years ago, it has limitations and it is not applicable to patients with turbulent flow in the Pulmonary Arteries (PA)[1]. We propose a new method to easily quantify the flow distribution in Fontan patients.

Systemic-to-pulmonary collateral flow in patients with palliated univentricular heart physiology: measurement using cardiovascular magnetic resonance 4D velocity acquisition

BACKGROUND Systemic-to-pulmonary collateral flow (SPCF) may constitute a risk factor for increased morbidity and mortality in patients with single-ventricle physiology (SV). However, clinical research is limited by the complexity of multi-vessel two-dimensional (2D) cardiovascular magnetic resonance (CMR) flow measurements. We sought to validate four-dimensional (4D) velocity acquisition sequen...

Aortopulmonary collaterals after bidirectional cavopulmonary connection or Fontan completion: quantification with MRI.

BACKGROUND Aortopulmonary collaterals (APCs) have been associated with increased morbidity after the Fontan operation. We aimed to quantify APC flow after bidirectional cavopulmonary connections and Fontan completions, using phase-contrast MRI, and to identify risk factors for the development of APCs. METHODS AND RESULTS APC blood flow was quantifiable in 24 of 36 retrospectively analyzed MRI...

Caval contribution to flow in the branch pulmonary arteries of Fontan patients with a novel application of magnetic resonance presaturation pulse.

BACKGROUND A complete understanding of fluid mechanics in Fontan physiology includes knowledge of the caval contributions to right (RPA) and left (LPA) pulmonary arterial blood flow, total systemic venous return, and relative blood flow to each lung. METHODS AND RESULTS Ten Fontan patients underwent cine MRI. Three cine scans of the pulmonary arteries were performed: (1) no presaturation puls...

In vivo quantification of blood mixing in single ventricle patients with Fontan circulation using 4D flow MRI

Background Single ventricle physiology (SVP) is one of the most severe forms of complex congenital heart disease (CHD). Patients undergo multiple surgical interventions including the Fontan procedure (caval venous return is routed directly to the pulmonary arteries). Despite the apparent success of the procedure, it is unclear why some patients develop ‘failing Fontan physiology’ while others r...

4D flow MRI assessment of right atrial flow patterns in the normal heart – influence of caval vein arrangement and implications for the patent foramen ovale

AIM To investigate atrial flow patterns in the normal adult heart, to explore whether caval vein arrangement and patency of the foramen ovale (PFO) may be associated with flow pattern. MATERIALS AND METHODS Time-resolved, three-dimensional velocity encoded magnetic resonance imaging (4D flow) was employed to assess atrial flow patterns in thirteen healthy subjects (6 male, 40 years, range 25-...
