BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/bcr-2014206599 DESCRIPTION Nicorandil is a nicotinamide ester commonly prescribed for management of angina. Since the late 1990s it has been linked with drug-induced ulceration. The current British National Formulary suggests that between 1 in 10 000 and 1 in 1000 cases may be affected. Initially lesions were reported in the buccal cavity, subsequently throughout the gastrointestinal (GI) tract and at the anus, with a recent association to parastomal breakdown. Cases involving fistulation to adjacent organs have also been described. Metabolites of nicorandil, nicotinic acid and nicotinamide have been suggested as factors in ulcer formation, particularly with higher doses. A 62-year-old woman on nicorandil 30 mg twice daily for angina with a history of peripheral vascular disease, systemic lupus erythematosus, chronic obstructive pulmonary disease and hypothyroidism presented with a rectovaginal fistula. She was on no other medications associated with GI complications. Flexible sigmoidoscopy demonstrated a rectovaginal fistula. Biopsies demonstrated only granulation tissue and ulcer slough. A loop colostomy was fashioned to treat the fistula. Two months postoperatively, the patient represented with significant peristomal ulceration. Nicorandil was considered as a potential causative agent and treatment was discontinued. No further biopsies were taken at this stage. Within a 2-month period the peristomal ulceration had healed. The images demonstrate the significance of the peristomal disease developed while on nicorandil (figure 1) and the subsequent improvement on cessation (figure 2). It would appear likely in this case that nicorandil has contributed to the patient’s presentation and subsequent events. This medication needs to be considered when patients are being considered for surgical intervention.

Letter: Nicorandil-Induced Peristomal Ulceration

Dear Sirs Nicorandil (Ikorel; Rhône–Poulenc Rorer, Guildford, UK) is a potassium-channel activator used in the treatment of severe ischaemic heart disease. It causes anal and oral ulceration. We report the case of the rare phenomenon of Nicorandil-induced peristomal ulceration, the severity of which almost required plastic surgery input. A 71-year-old lady, who had a Hartmann’s procedure 5 year...

A case of nicorandil-induced unilateral corneal ulceration.

Nicorandil, a second-generation nitro derivative, has been reported to induce single or multiple ulcerations in many locations, including oral, anal, perianal, vulvovaginal, perivulval, penile, gastrointestinal, colic, peristomal and skin locations. Ocular locations are now highly suspected. Herein, we report the case of a 78-year-old woman who experienced corneal ulceration at second cataract ...

Nicorandil-induced penile ulceration: a case report

BACKGROUND Penile ulceration in older patients is commonly neoplastic or infective. However, there are rarer causes of these ulcerations that we have to exclude. We present a rare complication of penile ulceration secondary to nicorandil, a nicotinamide ester. CASE PRESENTATION An 86-year-old white British man was referred with a bloody discharge from under his phimotic foreskin and a lump on...

Nicorandil as a Cause of Perineal Ulceration

Letters NicoraNdil as a cause of periNeal ulceratioN. editor, We report a case of extensive perineal ulceration that healed spontaneously on discontinuation of nicorandil therapy, avoiding major perineal reconstructive surgery. We note a previous report of penile ulceration related to nicorandil therapy in this journal and wish to remind readers to consider nicorandil as a causative agent for a...

Persistent nicorandil induced oral ulceration.

Four patients with nicorandil induced ulceration are described, and the literature on the subject is reviewed. Nicorandil induced ulcers are very painful and distressing for patients. Clinically they appear as large, deep, persistent ulcers that have punched out edges. They are poorly responsive to topical steroids and usually require alteration of nicorandil treatment. The ulceration tends to ...
