Multi-Method Approach for Medication Safety Event Detection in Community Hospitals
نویسندگان
چکیده
Background. Sharp HealthCare, in partnership with the University of Arizona College of Nursing, is conducting a study to evaluate the impact of a community hospital computerized provider order entry (CPOE) system on adverse drug event (ADE) outcomes. The medication safety arm of the study uses an observation methodology to detect medication safety events (MSE) that are subsequently evaluated to assess the study’s composite primary indicator: preventable ADEs plus non-intercepted potential ADEs (PADE), i.e., “near misses.” We report our preliminary findings for one of three study hospitals to illustrate the efficacy of our multimethod approach to MSE detection.

Advancing Opioid Safety for Children in Hospitals
The provision of medications to paediatric patients involves challenges distinct from those associated with medication administration to adult patients. These challenges arise for all paediatric age groups (infants, children, and youth) in every care setting, from community hospitals to specialty tertiary care facilities. Management of high-alert medications1, such as opioids, poses additional ...

This paper proposes a new mixed-integer model for the multi-skill resource-constrained project scheduling problem (MSRCPSP). The interactions between workers are represented as undirected networks. Therefore, for each required skill, an undirected network is formed which shows the relations of human resources. In this paper, community detection in networks is used to find the most compatible wo...
متن کاملDynamic Safety Analysis CNG Stations Using Fault Tree Approach and Bayesian Network
Introduction: The safety of CNG stations is important because of their location in urban areas, as well as to prevent accidents and to protect the safety of personnel, property, and environment. An event occurrence analysis with probability updating is the key to dynamic safety analysis. Methods and materials: In this study, the Failure Modes and Effects Analysis (FMEA) technique was used to d...
متن کاملChoosing appropriate theories for understanding hospital reporting of adverse drug events, a theoretical domains framework approach
Adverse drug events (ADEs) may cause serious injuries including death. Spontaneous reporting of ADEs plays a great role in detection and prevention of them, however, underreporting always exists. Although several interventions have been utilized to solve this problem, they are mainly based on experience and the rationale for choosing them has no theoretical base. The vast variety of behavioral ...

Introduction: In hospitals, an optimum drug dispensing (DD) is involved in the quality of health services and the prevention of medication errors. This study aims to describe the features of hospital DD in Cote d’Ivoire in order to suggest improvements. Methodology: The site visit (risk management method) was conducted at hospital pharmacies (HP) of the three teaching hospitals in Abidjan. A se...
