چکیده

TheAASM, ATS, andACCP agree that there is an enormous need for research to improve the understanding and treatment of sleep disorders. Thus, the three societies strongly advocate for increased research support in this critical area. In addition, the societies support initiatives to enhance the number of wellqualified researchers in the broad area of sleep medicine. The societies have agreed to sponsor a workshop in the coming year, which will include representatives from all concerned specialties, to explore means to foster research and identify potential areas for cooperative research. The field of sleep medicine is in a time of rapid growth and maturation. The ACCP, the ATS, and the AASM will continue to collaborate on initiatives that further the growth of the field, help our members, and above all, help our patients. The societies have agreed to explore ways to enhance quality of care and access, and to mount efforts to ensure adequate numbers of well-trained sleep technologists. We will also continue to discuss relevant issues in sleep medicine, and explore opportunities to enhance physician education and patient care. We urge all concerned with the future of sleep medicine to participate actively and collaboratively as we move forward together.

Differences in hospital glycemic control and insulin requirements in patients recovering from critical illness and those without prior critical illness

INTRODUCTION Hospital patients recovering from critical illness on general floors often receive insulin therapy based on protocols designed for patients admitted directly to general floors. The objective of this study is to compare glycemic control and insulin dosing in patients recovering from critical illness and those without prior critical illness. METHODS Medical record review of blood g...

Pro/con debate: Is intensive insulin therapy targeting tight blood glucose control of benefit in critically ill patients?

You have decided to develop a protocol for insulin therapy in your intensive care unit (ICU). You wonder about the merit of using intensive insulin therapy (IIT) to maintain tight blood glucose control in your patients.

Intensive insulin therapy protects the endothelium of critically ill patients.

The vascular endothelium controls vasomotor tone and microvascular flow and regulates trafficking of nutrients and biologically active molecules. When endothelial activation is excessive, compromised microcirculation and subsequent cellular hypoxia contribute to the risk of organ failure. We hypothesized that strict blood glucose control with insulin during critical illness protects the endothe...

Adiponectin, retinol-binding protein 4, and leptin in protracted critical illness of pulmonary origin

INTRODUCTION Critically ill patients requiring intensive care uniformly develop insulin resistance. This is most pronounced in patients with sepsis. Recently, several hormones secreted by adipose tissue have been identified to be involved in overall insulin sensitivity in metabolic syndrome-related conditions. However, little is known about these adipokines in critical illness. METHODS We stu...

Hyperglycemia and acquired weakness in critically ill patients: potential mechanisms

Critical illness polyneuropathy/critical illness myopathy (CIP/CIM) is a major cause of mortality and long-term morbidity in critically ill patients, but the true incidence and prevalence of these syndromes are not known. Hermans and colleagues show that when intensive insulin therapy is used as part of routine clinical practice in the intensive care unit, the incidence of CIP/CIM as determined...

An overview of hypoglycemia in the critically ill.

Hypoglycemia is a common and serious problem among patients with diabetes mellitus. It is also perceived as the most important obstacle to tight glucose control using intensive insulin therapy in critically ill patients. Because glucose is an obligatory metabolic fuel for the brain, hypoglycemia always represents an emergency that signals the inability of the brain to meet its energy needs. Whe...
