چکیده

This is a retrospective study. The purpose of this study was to assess the diagnostic accuracy and utility of posttransplant magnetic resonance cholangiography (MRC) in detecting biliary complications and planning the treatment. From July 2006 to April 2010, 51 (19%) of these 268 adult living-donor liver transplantation (LDLT) recipients were referred to MRC due to biliary dilatation and abnormalities detected by ultrasound or abnormal liver function. The biliary complications were treated with endoscopic or percutaneous transhepatic approach. Among those 51 recipients, MRC revealed no definite biliary pathology in 34 patients. In 10 of the 34 MRCnegative cases, additional MR angiography showed vascular stenosis. 15 MRC-positive cases were correctly assessed the site of the obstruction, bile leakage or lithiasis. Under MRC guidance, biliary interventions were completed in 12 of the MRC-positive cases. MRC achieved sensitivity of 100%, specificity of 94.4%, positive predictive value of 88.2%, and negative predictive value of 100%. MRC is a reliable diagnostic modality in detecting post-transplant biliary complications with 96.1% accuracy. It is an essential diagnostic tool for assessing the necessity for interventional procedure. MR angiography can provide additional information on vascular problems that caused biliary complications. Magnetic resonance imaging (MRI) is thus indispensable before therapeutic biliary or vascular procedure in post-transplant recipients. Microchimerism and Renal Transplantation: Doubt Still Persists

Background: The Presence of donor leukocytes in recipients of organ allograft has been shown even several years after transplantation. However, it remains unclear whether this donor cell microchimerism plays an effective role in allograft acceptance or is simply a consequence of immunosuppressive conditions in recipients. Objective: To study microchimerism in a group of kidney transplant recipi...

Surgical management of anatomical variations of the right lobe in living donor liver transplantation.

OBJECTIVE To review the anatomical variations of the right lobe encountered in 40 living liver donors, describe the surgical management of these variations, and summarize the results of these procedures. SUMMARY BACKGROUND DATA Anatomical variability is the rule rather than the exception in liver and biliary surgery. To make effective use of liver segments from living donors for transplantati...

Current management of biliary complications after liver transplantation: emphasis on endoscopic therapy.

Biliary complications occur in 5-25% of patients after liver transplantation and represent a major source of morbidity in this group of individuals. The major risk factor for most of these complications is ischemia of the bile tree usually due to obstruction or vascular insufficiency of the hepatic artery. The most common complications include biliary strictures (anastomostic and nonanastomotic...

Management of Biliary Strictures After Liver Transplantation.

Strictures of the bile duct are a well-recognized complication of liver transplant and account for more than 50% of all biliary complications after deceased donor liver transplant and living donor liver transplant. Biliary strictures that develop after transplant are classified as anastomotic strictures or nonanastomotic strictures, depending on their location in the bile duct. The incidence, e...
