چکیده
We report a case of 61-year-old man presented with progressive dyspnea for 5 month followed by respiratory failure. Transthoracic echocardiogram revealed bacterial vegetation, rheumatic change mitral valve and destruction of the aortic valve. Initial hemoculture reported gram-positive cocci in chain for two specimens and its resistance to penicillin and vancomycin. Daptomycin was administered as a primary drug, but the latter hemoculture was identified as Abiotrophia defectiva. Infectious endocarditis caused by A. defectiva was diagnosed, he was treated with diuretic and penicillin G. Aortic valve replacement and valvulopathy of mitral valve was done due to congestive heart failure and large vegetation. After operation and total penicillin G 4 weeks, he improved and was discharged from the hospital. Infectious diseases caused by A. defectiva are extremely rare, and identification of this pathogen is important, as its bacterial characteristics require proper attention. (J Infect Dis Antimicrob Agents 2012;29:21-5.) Note: This case was presented in the Interhospital Case Conference on Infectious Diseases (ICCID), 18 August 2011, Bangkok, Thailand. INTRODUCTION Abiotrophia species, first described as nutritionally variant (deficient) streptococci in 19611, were originally discovered as small satellite colonies around coagulasenegative bacteria or with supplementation of complex media with cysteine or pyridoxal.2 Taxonomic studies of the Abiotrophia spp. concluded that this group of organisms should be reclassified separately into Abiotrophia defectiva and Granulicatella adiacens, G. balaenopterae, and G. elegans.3 Although Abiotrophia/Granulicatella are parts of the normal flora of the oral cavity, the genitourinary tract, and the intestinal tract, the pathogenic potential of these organisms has been well established. They have been estimated to cause approximately 5-6% of microbiologically proven cases of endocarditis4, Division of Infectious Diseases, Department of Medicine, Faculty of Medicine Ramathibodi Hospital, Mahidol University, Bangkok 10400, Thailand. RATIONALE Endocarditis with Abiotrophia defectiva represents 4.3% to 6% of all streptococcal endocarditis. The article presents diagnosis issues and the complexity of the treatment. PATIENT CONCERNS We present the case of a female white patient, aged 26 years, who developed infectious endocarditis caused by A defectiva, in the last trimester of pregnancy, a biofilm-related infection associate...
متن کاملAbiotrophia defectiva causing infectious crystalline keratopathy and corneal ulcer after penetrating keratoplasty: a case report
BACKGROUND Infectious crystalline keratopathy is commonly caused by Streptococcus viridans and other gram positive organisms. We present the first case of infectious crystalline keratopathy that developed into a corneal ulcer and grew Abiotrophia defectiva which responded well to topical and systemic antimicrobial therapy and did not require re-grafting. A 78-year-old man underwent penetrating ...

UNLABELLED Abiotrophia defectiva or nutritionally variant Streptococcus (NVS) are a rare but important cause of infectious endocarditis, with high rates of bacteriological failure and mortality. We report the case of a 74-year-old man admitted for fever, fatigue and general malaise in the absence of any underlying cardiac, immunosuppressive illness and previous dental manipulations. Transthorac...
متن کاملA Case of Infective Endocarditis caused by Abiotrophia defectiva in Korea
Abiotrophia defectiva, a nutritionally variant streptococci can cause bacteremia, brain abscess, septic arthritis and in rare cases, infective endocarditis, which accounts for 5-6% of all cases. A. defectiva is characteristically difficult to diagnose and the mortality, morbidity and complication rates are high. Here, we discuss a case of infective endocarditis caused by A. defectiva. A 62-year...
متن کاملAbiotrophia species as a cause of endophthalmitis following cataract extraction.
Microorganisms of the genus Abiotrophia, members of the oral flora, are known as important causes of bacterial endocarditis. In this study, we report two individual cases of acute vitreous infection caused by Abiotrophia adiacens and Abiotrophia defectiva approximately a week after cataract extraction. Abiotrophia isolates were recovered by cultivation of vitreous humor on chocolate agar and id...
