Reversal of white matter edema in hypertensive encephalopathy.

چکیده
A 42-year-old man was admitted to the hospital because of increasing difficulty with gait and recent loss of vision. Examination revealed a blood pressure of 250/150 mm Hg, normal mental status , and severe hypertensive retinopathy bilaterally. There was no papilledema, and the fundi were pale. Cranial nerve, motor, and sensory examinations were normal. Plantar reflexes were extensor bilaterally. Gait was wide-based and unstable. There was no dysmetria on fingerto-nose testing. Electrolytes were remarkable for a potassium of 2.6 , a blood urea nitrogen of 75, and a creatinine of 3.8. CT showed obliteration of the fourth ventricle and marked narrowing of the prepontine cistern; patchy periventricular hypodensity was present supratentorially. A subsequent MR study (Figs. 1 A and 1 C) suggested diffuse white matter edema, particularly in the posterior fossa. The patient 's gait improved dramatically, and his plantar responses became flexor after vigorous treatment of his hypertension. Repeat CT scan at 1 week showed resolution of mass effect in the posterior fossa. Four days later, a follow-up MR study also showed improvement (Figs. 1 B and 1 D).

An unsual finding of brain magnetic resonance imaging in a hypertensive patient
Brain edema in patients with hypertensive encephalopathy frequently affects the parietooccipital white matter. Hypertensive encephalopathy is thus included as a differential diagnosis in reversible posterior leukoencephalopathy syndrome. Diffuse white matter involvement rarely occurs. We report a 41-year old woman with hypertensive encephalopathy with diffuse and non-enhancing white matter hype...
متن کاملHypertensive encephalopathy involving the brainstem and deep structures: a case report.
Hypertensive encephalopathy rarely presented with widespread edema in the cerebral white matters, deep structures and whole brainstem. A 80-year-old woman manifested as high arterial blood pressure, visual disturbance, severe headache, nausea, and vomiting. T2-weighted and fluid-attenuated inversion recovery magnetic resonance imaging showed high signal-intensity lesions in the cerebral white m...
متن کاملDiffusion-weighted MR imaging in hypertensive encephalopathy: clues to pathogenesis.
PURPOSE Hypertensive encephalopathy, a complex of cerebral disorders, including headache, seizures, visual disturbances, and other neurologic manifestations, is associated with a variety of conditions in which blood pressure rises acutely. It has been ascribed to either exuberant vasospasm with ischemia/infarction or breakthrough of autoregulation with interstitial edema. Diffusion-weighted MR ...
متن کاملHypertensive encephalopathy extending into the whole brainstem and deep structures.
In patients with hypertensive encephalopathy, brain edema is frequently distributed in the parieto-occipital white matter. We report a patient with high arterial blood pressure of over 300/160 mmHg on admission, who had extensive MRI-documented reversible lesions throughout the whole brain, including the brainstem, thalami, basal ganglia, and cerebellum. Extraordinarily severe acceleration of h...
متن کاملReversible Confluent Deep White Matter Abnormalities: A New Variant of Posterior Reversible Encephalopathy Syndrome
We describe a confluent deep white matter abnormalities variant of PRES, further strengthening the notion that PRES is a disorder of radiological heterogeneity. We present 2 cases of PRES with findings of diffuse but reversible vasogenic edema located in the deep periventricular white matter regions of bilateral hemispheres without a clearly posterior distribution. We feel that this represents ...
