The abandonment in utero of the placenta in prolonged twin pregnancy after premature rupture of membranes

چکیده
Correspondence: Dr. Roxana Elena Bohîlţea e-mail: r.bohiltea@ yahoo.com Premature rupture of fetal membranes is associated with a substantial fetal morbidity and mortality. Immediate actions that can improve the future neonatal outcome are the administration of antenatal corticosteroid therapy, infection prophylaxis, tocolytic drugs and insurance of an appropriate neonatal nursery level. One of the essential points in the management of these cases is the attempt to extend as much as possible the gestational age, especially in mid trimester premature rupture of membranes and even more considering that the duration of the latency period inversely correlates with gestational age at membrane rupture. We present the case of a patient aged 41 years admitted as obstetric emergency for suspected leakage of clear fluid in context of dichorionic, diamniotic twin pregnancy, 22 weeks of gestation, obtained by in vitro fertilization due to primary infertility. Under continuous tocolytic and antibiotic treatment, the extension of the gestational age of the first fetus with 5 weeks was successfully obtained. The second fetus was born one week later as a result of the abandonment in utero of the placenta. Complications implied by preterm rupture of membranes in twin pregnancies are significantly influenced by the presence of chorioamnionitis and by placentation. We consider that the prognosis of the second fetus, perfectly normal developed during first 5 years, was positive and significantly influenced by the prolonged gestational age with almost one week.

Effects of Progesterone on Latency Period in Patients with Preterm Premature Rupture of Membranes during 24-34 Weeks of Pregnancy
Background & Aims: Preterm premature rupture of membranes (PPROM) is the leading cause of preterm birth and perinatal mortality and morbidity, and complications. The objective of the present study was to assess the effect of progesterone agents (rectal suppositories and weekly injection of 17- hydroxyprogesterone caproate) on latency period in women with PPROM. Methods: This randomized, double-...

Background: Vitamin D deficiency is common during pregnancy and childbirth. Recent findings suggest that vitamin D deficiency is associated with adverse pregnancy outcomes. This study was performed to determine the association between low levels of maternal serum vitamin D and some adverse pregnancy outcomes such as premature rupture of embryonic membranes. Materials and Methods: 73 pregnant w...
متن کاملTwin pregnancy – physiology, complications and the mode of delivery
The aim of the article is to present actual knowledge concerning twin pregnancy. After the physiological changes observed during such pregnancy, main complications such as intrauterine growth restriction (IUGR), prematurity, premature rupture of membranes (PROM) and hypertension are discussed. The paper ends with discussion about the possible routes of delivery in twin pregnancy.

Background: Approximately 3% to 5% of obstetric patients will experience postpartum hemorrhage (PPH). Even though the most common reason for postpartum hemorrhage, as the main cause of maternal death, is uterine atony; other complications such as laceration, hematoma, inversion, rupture; retained tissue or invasive placenta; and coagulopathy may result in PPH. The main cause of retained placent...

Bacterial vaginosis (BV) has been considered to be the most prevalent infection found in sexually active women. BV is thought to play an important role in the premature rupture of membranes (PROM) and preterm birth. Preterm delivery accounts for a substantial percentage of low birth weight infants and perinatal mortality and morbidity. Bacteroides and Prevotella species have been isolated f...
