چکیده

Jongman J, et al. BMJ Case Rep 2017. doi:10.1136/bcr-2017-219775 DESCRIPTION A 65-year-old patient with no medical history of notice was referred to our cardiology outpatient clinic for the evaluation of exertional dyspnoea. After a thorough work-up, the suspicion of an amyloid cardiomyopathy was raised by the cardiac magnetic resonance (CMR) findings (figure 1A–C), but laboratory tests, abdominal fat pad and bone marrow biopsies failed to prove the diagnosis. Eventually endomyocardial biopsies confirmed the diagnosis of secondary (AA) amyloidosis. Furthermore, the CMR scout images unveiled a large abdominal mass (figure 1D), with subsequent CT being suggestive of a pancreatic neuroendocrine tumour with multiple hepatic metastases. Clinically apparent heart disease or cardiac amyloidosis is uncommon in secondary amyloidosis. Secondary amyloidosis may complicate chronic inflammatory diseases and is rarely associated with other causes, for example, neoplasms. In the diagnosis of cardiac amyloidosis, the characteristic echocardiographic finding of an increased echogenicity of the myocardium has limited specificity and sensitivity. In contrast, CMR imaging with a distinctive late gadolinium enhancement pattern can provide evidence strongly suggestive of cardiac amyloid deposition and can be used to guide myocardial biopsies, which have a higher sensitivity compared with subcutaneous fat or rectal biopsies. 3 In our patient, the characteristic CMR findings proved to be pivotal for the diagnosis of the secondary or AA amyloidosis with cardiac manifestation. Furthermore, the CMR scout images unveiled an important incidental finding, that is, a large abdominal mass, possibly associated with the amyloidosis in this patient.

Familial Amyloid Polyneuropathy Type IV (FINNISH) with Rapid Clinical Progression in an Iranian Woman: A Case Report

Familial amyloid polyneuropathy (FAP) type IV (FINNISH) is a rare clinical entity with challenging neuropathy and cosmetic deficits. Amyloidosis can affect peripheral sensory, motor, or autonomic nerves. Nerve lesions are induced by deposits of amyloid fibrils and treatment approaches for neuropathy are challenging. Involvement of cranial nerves and atrophy in facial muscles is a real concern i...

PRIMARY LOCALIZED RETROPERITONEAL AMYLOID TUMOR WITH OSSEOUS META PLASIA: CASE REPORT AND REVIEW OF THE LITERATURE

Amyloidoma or amyloid tumor is a tumor-like localized deposit of amyloid encountered occasionally in association with multiple myeloma, various chronic inflammatory diseases and primary amyloidosis. Amyloid tumors unassociated with plasmacytoma or other disease are extremely rare in soft tissues and few cases in various situations have been reported. Histologic examination discloses amorp...

Amyloid Light-Chain Amyloidosis Manifesting as Heart Failure with Preserved Ejection Fraction in a Patient with Hyper-Immunoglobulin E-emia.

BACKGROUND Considering the increased prevalence of heart failure with preserved ejection fraction (HFpEF) as a result of the aging population, the pathophysiology of HFpEF needs to be examined. Furthermore, many comorbidity profiles in patients with HFpEF have been reported. Hypertrophic cardiomyopathy is a well-known specific etiology of HFpEF. Cardiac amyloidosis, which mimics infiltrative an...

Immunoglobulin light-chain (AL) amyloidosis with myasthenic symptoms and echocardiographic features of dilated cardiomyopathy.

Myasthenic symptoms and the echocardiographic findings of dilated cardiomyopathy are very rare in primary AL amyloidosis. We report a 59-year-old man with dyspnea on effort and weakness after exercise. His electrocardiogram showed ischemic heart disease and echocardiography indicated dilated cardiomyopathy. Muscle biopsy revealed amyloidosis with deposits of lambda light chain-derived amyloid w...
