چکیده

BACKGROUND Dynamic hyperinflation, caused by expiratory flow limitation, markedly increases resting end-expiratory lung volume (functional residual capacity) in many COPD patients. OBJECTIVE To determine the impact and duration of impact of CPAP on hyperinflation and airway resistance in patients with stable COPD. METHODS In a case series, 21 patients underwent CPAP at 8 cm H(2)O for 15 min, then whole-body plethysmography immediately after, and at 15 and 30 min after CPAP. RESULTS The cohort's mean ± SD age was 70 ± 9 y, and the mean FEV(1) was 41 ± 8% of predicted. Residual volume, functional residual capacity, total lung capacity, the ratio of residual volume to total lung capacity, and airway resistance decreased after CPAP and did not significantly change at 15 min (P < .001), but returned to baseline at 30 min. CONCLUSIONS In patients with severe to very severe stable COPD, CPAP reduces lung volumes and airway resistance for 15 min, but the lung volumes return to baseline by 30 min.

Efficacy of noninvasive CPAP in COPD with acute respiratory failure.

Dynamic hyperinflation and the development of intrinsic positive end-expiratory pressure (PEEPi) are commonly observed in patients with severe chronic obstructive pulmonary disease (COPD) in acute respiratory failure. Previous studies have shown that externally applied PEEP reduces PEEPi and its adverse effects in mechanically-ventilated COPD patients. The purpose of this study was to determine...

Inspiratory fraction correlates with exercise capacity in patients with stable moderate to severe COPD.

BACKGROUND Exercise intolerance is the hallmark of COPD. Static lung hyperinflation and increased dynamic hyperinflation during exercise are associated with reduced functional capacity in COPD patients. Inspiratory capacity correction for the total lung capacity, defined as inspiratory fraction (IF), may be functionally more representative than other traditional indices in these patients. OBJ...

"Intrinsic" PEEP (PEEPi): role of expiratory muscles.

In normal subjects breathing at rest, lung volume at end-expiration corresponds with the elastic equilibrium volume, or relaxation volume (Vr), of the respiratory system. If expiratory duration is not long enough, then inspiration may begin before the system has returned to Vr, such that end-expiratory lung volume is higher than Vr. This condition is named dynamic pulmonary hyperinflation and "...

Effects of Heliox in Stable COPD Patients at Rest and during Exercise

Heliox has been administered to stable chronic obstructive pulmonary disease (COPD) patients at rest and during exercise on the assumption that this low density mixture would have reduced work of breathing, dynamic hyperinflation, and, consequently, dyspnea sensation. Contrary to these expectations, beneficial effects of heliox in these patients at rest have been reported only sporadically, and...
