Impaction grafting for acetabular revision: bringing back the bone.

چکیده
Impaction grafting has been widely promoted on the acetabular side by Schreurs et al,1 with associated bone grafting and a cemented cup. The objectives of impaction grafting are to: 1) transform segmental defects into cavitary defects, 2) obtain a full compaction of the graft, 3) achieve primary stability of the cup, which is anatomically located in the acetabulum, and 4) restore bone stock. This technique can be combined with the use of inner-medial or rim mesh according to the type of defect. Impaction grafting can be used in cementless fixation with morsellized bone graft and a hemispherical cup.2,3 However, in some cases, primary stability of the cement-

UNLABELLED Although uncemented cup implants frequently are used in young patients, we believe long-term survival rates of cups in these patients are somewhat disappointing, and therefore we have continued to use cemented cups in primary THA, even in young patients. However, in cases of acetabular bone stock defects, we also use bone impaction grafting. We prospectively followed 130 patients wit...
متن کاملImpaction grafting of the acetabulum with ceramic bone graft substitute
BACKGROUND AND PURPOSE Loss of bone stock remains a challenge in revision hip surgery. Grafting with allograft is well established, but there are problems with availability, cost, infection, antigenicity, reproducibility, and stability of the created construct. BoneSave is a biphasic porous ceramic consisting of sintered 80% tricalcium phosphate and 20% hydroxyapatite. In vitro and in vivo stud...
متن کاملImpaction bone grafting of the acetabulum at hip revision using a mix of bone chips and a biphasic porous ceramic bone graft substitute
BACKGROUND AND PURPOSE One of the greatest problems of revision hip arthroplasty is dealing with lost bone stock. Good results have been obtained with impaction grafting of allograft bone. However, there have been problems of infection, reproducibility, antigenicity, stability, availability of bone, and cost. Thus, alternatives to allograft have been sought. BoneSave is a biphasic porous cerami...
متن کاملAcetabular impaction bone grafting in total hip replacement.
The increasing need for total hip replacement (THR) in an ageing population will inevitably generate a larger number of revision procedures. The difficulties encountered in dealing with the bone deficient acetabulum are amongst the greatest challenges in hip surgery. The failed acetabular component requires reconstruction to restore the hip centre and improve joint biomechanics. Impaction bone ...
متن کاملAcetabular Reconstruction with Massive Allograft Shaped to the Cavity and Kerboull-Type Acetabular Reinforcement Device for Multiple Failures of Impaction Bone Graft: A Case Report
Acetabular component revision in the presence of severe bone loss is difficult for reconstructing an acetabular component in the anatomical hip center. Various treatment options are available, although often the acetabular defect cannot be corrected with a single option alone. Precise assessment of the bone loss and a suitable combination of methods are needed. Here we report a case of multiple...
